Responsibilities:
- Conduct assessments to determine client needs and develop individualized service plans.
- Provide counseling and support to clients dealing with mental health, social, and emotional challenges.
- Advocate for clients and connect them with appropriate community resources and services.
- Collaborate with healthcare providers, government agencies, and community organizations to coordinate client care.
- Maintain detailed and confidential client records in compliance with organizational policies and regulations.
- Educate clients and their families about coping strategies, available services, and support networks.
- Participate in case reviews, team meetings, and ongoing professional development.
Qualifications:
- Master’s Degree from an accredited institution.
- Current LPC/LCSW license in Georgia.
- Strong knowledge of social work principles, ethics, and best practices.
- Excellent communication, organizational, and problem-solving skills.
- Ability to work independently and as part of a multidisciplinary team.
- Proficiency in Electronic Health Records (EHR) software/other relevant software.
- Bilingual Spanish preferred but not required.
