Responsibilities:

  • Conduct assessments to determine client needs and develop individualized service plans.
  • Provide counseling and support to clients dealing with mental health, social, and emotional challenges.
  • Advocate for clients and connect them with appropriate community resources and services.
  • Collaborate with healthcare providers, government agencies, and community organizations to coordinate client care.
  • Maintain detailed and confidential client records in compliance with organizational policies and regulations.
  • Educate clients and their families about coping strategies, available services, and support networks.
  • Participate in case reviews, team meetings, and ongoing professional development.

Qualifications:

  • Master’s Degree from an accredited institution.
  • Current LPC/LCSW license in Georgia.
  • Strong knowledge of social work principles, ethics, and best practices.
  • Excellent communication, organizational, and problem-solving skills.
  • Ability to work independently and as part of a multidisciplinary team.
  • Proficiency in Electronic Health Records (EHR) software/other relevant software.
  • Bilingual Spanish preferred but not required.
