Employment Type: Per Diem, Non-Exempt
Location: Hybrid, MN (Mostly remote, required to be on site roughly once a month)
Compensation: Starting at $35.00/hour, based on experience

About the Licensed Practical Nurse (Per Diem) Role:
The Licensed Practical Nurse (Per Diem) will provide clinical, clerical, and technical support to assist physicians (MD/DO) and Advanced Practice Clinicians (NP/PA) in the primary care setting. The LPN will play a vital role in supporting Transitional Care Management (TCM) by helping patients navigate post-discharge follow-up, medication reconciliation, and ongoing care needs. The LPN will contribute to ensuring continuity of care and positive outcomes for patients during critical transitions, while also providing hands-on patient support in the clinic.
Core Responsibilities:
- Support Transitional Care Management (TCM) efforts, including patient outreach, follow-up scheduling, and medication reconciliation
- Enter and update data in the EMR, including prepping for preventive care and overdue screenings
- Review and update medication lists, prepare orders for refills and signatures by the provider
- Assist with chronic disease management and population health efforts, including outreach calls and medication adherence follow-ups
- Maintain open, proactive communication with providers about patient care opportunities and needs
Core Qualifications:
- Active LPN license in the state of Minnesota
- Concierge-level customer service and work ethic
- Excellent interpersonal and communication skills
- Strong organizational skills and attention to detail
- Ability to work effectively in a flexible, innovative, and fast-paced environment
- Fast, reliable internet connection
Additional Qualifications:
- Passage of the pre-employment drug screen
- Passage of the pre-employment background check
- Eligibility and completion of employee references
- Proof of US Citizenship or Eligibility to Work in the US with a permanent resident card
