Pharmacist (Part-Time Contract)

The Opportunity
We are seeking a Pharmacist to join our team. In this role, you will provide critical medication management services to seniors in rural communities, ensuring safe and effective use of medications. You will work closely with our care teams to optimize medication regimens, monitor for side effects, and educate patients on their treatment plans.
What You'll Do
- Medication management: Review and monitor patient medications to ensure they are safe, effective, and appropriate for the patient’s health condition
- Collaborate with physicians, nurses, and other healthcare providers to optimize medication regimens for patients with complex health conditions
- Monitor for potential medication side effects, adverse reactions, and drug interactions, and adjust treatment plans accordingly
- Provide patient education on medication usage, potential side effects, and proper adherence to prescribed treatments.
- Work closely with patients and their caregivers to ensure understanding and adherence with medication plans, empowering them to make informed decisions
- Communicate regularly with the care team to ensure all medications align with the patient's overall care plan and health goals
- Ensure documentation of medication management interventions and adherence to regulatory standards.
- Assist in identifying cost-effective medication alternatives for patients, considering their unique circumstances and resources
- Stay current on pharmaceutical research and trends, particularly those related to geriatric care and rural healthcare
What You Bring
- Doctor of Pharmacy (PharmD) degree from an accredited institution
- Licensed Pharmacist in Minnesota, with additional licensure in Michigan preferred
- 3+ years of experience in medication management, with a focus on elderly populations
- Strong understanding of geriatric pharmacotherapy and the unique needs of seniors
- Experience in collaborative care settings, working with physicians, nurses, and other healthcare professionals to deliver comprehensive care
- Strong communication and patient education skills, with a passion for helping patients understand their medications and treatment plans
- Ability to think critically and problem-solve when it comes to medication-related issues, such as side effects, drug interactions, and patient compliance

- Experience working in a startup or fast-paced environment
- Previous experience working with Medicare populations
- Experience working in rural or underserved healthcare settings
- A creative, problem-solving mindset when addressing medication-related challenges in resource-limited settings
