The Opportunity

The Primary Care Service Line Leader will oversee the strategic direction, operations, and performance of Homeward’s Primary Care Service Line. This leader will focus on delivering supplemental primary care services to complement existing PCP relationships for Homeward members, ensuring alignment with value-based care goals and optimal population health outcomes. The role requires expertise in care delivery, team leadership, and collaboration with external providers and internal teams to drive improved member health outcomes while avoiding disruption to members’ established care relationships.


What You'll Do

Service Line Leadership and Performance Management:

  • Oversee the strategic vision and operations of the Primary Care Service Line, ensuring alignment with Homeward’s objectives.
  • Define and monitor key performance indicators (KPIs) related to supplemental primary care delivery, member outcomes, and operational efficiency.
  • Manage and mentor a team of Advanced Practice Providers (APPs) and clinical support staff, ensuring high-quality, member-centered care.

Supplemental Primary Care Model Development:

  • Design and implement workflows to act as a supplemental primary care provider (PCP) for members, focusing on addressing gaps in care and unmet needs.
  • Collaborate with members’ existing PCPs to enhance continuity of care and avoid duplication of services.
  • Develop protocols to integrate pre-visit assessments, care gap closures, and screenings into supplemental care visits.

Operational Oversight:

  • Ensure care delivery is efficient and scalable, leveraging centralized processes and tools where appropriate.
  • Drive care team adherence to clinical protocols, quality standards, and regulatory requirements.
  • Implement strategies to identify and address high-priority clinical needs within the member population.

Collaboration and Integration:

  • Partner with other Service Line Leaders, including Navigation and Care Management, to provide holistic, coordinated care for members.
  • Establish strong communication channels with external PCPs and specialists to enhance collaboration and care alignment.
  • Work with health plan partners to support care gap closure initiatives and quality improvement programs.

Technology and Process Optimization:

  • Leverage telehealth and remote monitoring technologies to enhance access and engagement with members.
  • Utilize data analytics and automation to identify care gaps, streamline workflows, and prioritize high-need members.
  • Continuously evaluate and refine care delivery processes to improve efficiency and outcomes.

Member Engagement and Advocacy:

  • Develop and execute strategies to engage members in supplemental primary care services, ensuring high satisfaction and adherence to care plans.
  • Act as a member advocate, addressing barriers to care and providing personalized, culturally appropriate services.

Skills and Competencies

  • Expertise in primary care delivery, care coordination, and addressing care gaps.
  • Strong leadership and team management skills to oversee a distributed clinical workforce.
  • Proficiency in telehealth, remote monitoring, and EMR systems to support supplemental care delivery.
  • Exceptional collaboration and relationship-building abilities with internal teams and external PCPs.
  • Data-driven decision-making skills with the ability to analyze clinical and operational trends.
  • Deep understanding of value-based care models, population health strategies, and quality improvement programs.

 

What Else?

  • Advanced degree in Nursing, Medicine, Healthcare Administration, or a related field (e.g., MSN, DNP, MD, or equivalent).
  • Active clinical license (e.g., Nurse Practitioner, Physician Assistant, or Physician) preferred but not required.
  • 5+ years of experience in primary care delivery or clinical operations, with at least 2 years in a leadership role.
  • Experience working within value-based care or population health initiatives.

WOW Factor

  • Background in supplemental or collaborative care models, particularly in rural or underserved populations.
  • Experience managing telehealth or hybrid care delivery models.
  • Certification in quality improvement or care coordination (e.g., PCMH, NCQA).

The base salary range for this position is $150,000 - $185,000 annually. Compensation may vary outside of this range depending on a number of factors, including a candidate’s qualifications, skills, location, competencies and experience. Base pay is one part of the Total Package that is provided to compensate and recognize employees for their work at Homeward Health. This role is eligible for an annual bonus, stock options, as well as a comprehensive benefits package.
