The Opportunity

Join us in tackling healthcare for rural America! We’re seeking a full-time Physician who is passionate about helping people in rural communities throughout Minnesota. This role offers a unique opportunity to provide high-quality, patient-centered care while supporting and supervising Advanced Practice Providers (APPs).

What You'll Do

  • Provide wrap-around and collaborative primary care services to adult patients in an outpatient setting.
  • Conduct routine physical exams, wellness visits, and screenings in alignment with current guidelines.
  • Manage acute and chronic conditions, ensuring evidence-based, patient-centered treatment plans.
  • Assume care for the most medically complex patients, co-managing them with APPs to ensure comprehensive treatment and optimal patient outcomes.
  • Coordinate care by effectively communicating with patients' primary care providers and specialists, ensuring seamless transitions of care and a collaborative approach to treatment.
  • Order and interpret diagnostic tests, making appropriate referrals to specialists when necessary.
  • Educate patients on treatment plans, medications, and lifestyle modifications to improve health outcomes.
  • Address patient concerns and respond to medical issues as needed.
  • Perform minor procedures and wound care as appropriate.
  • Follow all departmental policies and procedures to ensure compliance with clinical and regulatory standards.
  • Assist the Regional Medical Director in developing clinical strategy that aligns with key performance indicators (KPIs) to drive exceptional medical outcomes for our patients.

What You Bring

  • MD or DO
  • Active license in good standing in Minnesota, ideally with active license in Michigan as well
  • Board certified or eligible in Family Medicine or Internal Medicine
  • Active DEA license
  • Passion for delivering care in rural America
  • Experience working on a small fast-pace team is a plus
  • Experience working with connected devices/loT is a plus
  • Experience with medical or health data is a plus
  • Experience with various EHR platforms (Epic, Cerner, Athena, etc)

The base salary range for this position is $200,000-$300,000 annually. Compensation may vary outside of this range depending on a number of factors, including a candidate’s qualifications, skills, location, competencies and experience. Base pay is one part of the Total Package that is provided to compensate and recognize employees for their work at Homeward Health. This role is eligible for an annual bonus, stock options, as well as a comprehensive benefits package.
