The Opportunity

We’re looking for an experienced Risk Adjustment Educator to join our growing team and support provider practices across Michigan. As an In-Market Educator, you’ll deliver tailored documentation and coding education to contracted providers participating in Homeward’s External Risk Adjustment Enablement Program. Your goal: ensure providers understand and apply CMS risk adjustment documentation best practices to accurately reflect the clinical complexity of the patients they serve.

You will work closely with our coding team, practice partners, and payer collaborators; this role is ideal for someone who thrives at the intersection of clinical detail, provider engagement, and healthcare performance improvement.

What You'll Do

  • Provider Education & Coaching
    • Deliver tailored education sessions to external providers based on chart review findings, scorecard data, and payer feedback.
    • Conduct one-on-one coaching, small group huddles, and live or virtual trainings.
    • Translate CMS-HCC and risk adjustment coding requirements into practical, provider-friendly documentation guidance.

  • Documentation Review & Feedback
    • Support analysis of encounter documentation and coding quality to identify gaps or improvement opportunities.
    • Partner with coders and the Provider Enablement Senior Manager to develop actionable feedback for providers.
    • Reinforce documentation techniques aligned with Diagnosis/Status/Plan (DSP) methodology.

  • Payer-Aligned Program Execution
    • Help providers understand and meet requirements for plan-specific programs.
    • Guide providers in addressing chronic condition gaps, applying documentation best practices, and capturing diagnosis accuracy.

  • Practice Relationship Management
    • Build strong, trusting relationships with providers and practice administrators.
    • Serve as a local point of contact to triage questions related to risk adjustment, documentation, and program performance.
    • Travel to practices across Michigan, supporting on-site or hybrid models depending on need.

  • Continuous Improvement
    • Document key learnings and feedback from the field to inform future education tools and strategies.
    • Contribute to development of new content (e.g., tip sheets, micro-learnings, webinar content) in partnership with coding and product teams.

What You Bring

  • Ability to travel throughout the state of Michigan approximately 50% of the time
  • 3+ years of experience in provider education, clinical documentation improvement (CDI), or risk adjustment coding.
  • Certified Risk Adjustment Coder (CRC), CDIP, CCS, or related credential strongly preferred.
  • Strong working knowledge of Medicare Advantage, CMS-HCC guidelines, and ICD-10-CM coding.
  • Experience educating clinicians or clinical teams with empathy, clarity, and results orientation.
  • Ability to analyze clinical documentation and deliver structured, constructive feedback.
  • Strong interpersonal skills and experience building provider-facing relationships.
  • Comfortable navigating EMRs and clinical documentation systems.

Bonus Points

  • Experience supporting external networks or providers under value-based contracts.
  • Familiarity with BCBSM PEC or other Medicare Advantage payer incentive programs.
  • Experience with EMR-integrated documentation templates or coding tools.
  • Experience as a clinical instructor, HIM educator, or outpatient CDI specialist.

The base salary range for this position is $55,000 - $78,000 annually. Compensation may vary outside of this range depending on a number of factors, including a candidate’s qualifications, skills, location, competencies and experience. Base pay is one part of the Total Package that is provided to compensate and recognize employees for their work at Homeward Health. This role is eligible for an annual bonus, stock options, as well as a comprehensive benefits package.
