The Opportunity

Join us in tackling healthcare for rural America. The Physician Lead serves as the primary clinical authority for Care Delivery, with responsibility for clinical quality, documentation integrity, appropriate utilization, and provider performance within a value-based care model.

The Physician Lead plays a key role in advancing our mission to deliver accessible, effective care to rural populations. As the collaborating physician for our team of Advanced Practice Practitioners, this individual provides clinical oversight to support consistent, high-quality care. The Physician Lead partners closely with operational leadership on provider performance but does not have direct responsibility for employment decisions or compensation.

This is a part-time position, requiring approximately 20–30 hours per week.

 

What You’ll Do

Clinical Leadership & Quality Oversight

  • Set and uphold clinical standards aligned with evidence-based and value-based care
  • Identify and mitigate clinical quality and safety risks
  • Participate in clinical governance and quality reviews, including safety event debriefs

Provider Oversight & Performance Management

  • Provide clinical oversight for Care Delivery providers
  • Conduct chart audits to assess documentation quality, diagnostic accuracy, risk stratification, and utilization
  • Establish audit standards and deliver clear, case-based feedback
  • Intervene on outlier care or documentation and escalate persistent performance concerns to operational leadership
  • Serve as the designated escalation physician for complex clinical decision-making, with on-call responsibilities limited to escalation coverage
  • Provide real-time clinical support and guidance, including visit shadowing.

Documentation & Coding Integrity

  • Ensure accurate, defensible clinical documentation and coding practices
  • Support quality gap closure and appropriate CAP assessments
  • Translate coding and CDI requirements into clear expectations for clinicians
  • Identify and address systemic documentation risks and trends impacting quality, compliance or revenue integrity
  • Serve as the clinical escalation point for documentation disputes or concerns.

Payer Engagement & Utilization  

  • Serve as the physician representative for payor-facing clinical discussions.
  • Support medical necessity reviews, utilization alignment, and payer audits.
  • Ensure clinical documentation supports defensible utilization decisions.
  • Partner with internal teams on payor inquiries, audits, or escalations.

Drive Clinical Performance Metrics

  • Documentation quality and audit performance
  • Appropriate utilization (including ED avoidance)
  • Provider clinical performance
  • Payer audit outcomes related to clinical defensibility

Cross-Functional Leadership

  • Provide clinical input into program design, pilots, and workflows 
  • Ensure tools and processes are clinically sound, usable, and aligned with care standards
  • Model and support adoption of clinical technology tools

What You Bring:

  • MD or DO with an active medical license; board certification in family medicine, internal medicine, or another primary care specialty strongly preferred
  • Must be licensed in Michigan 
  • 2+ years of clinical experience in a value-based care setting, and demonstrated clinical leadership responsibility
  • Experience overseeing providers with strong documentation, CDI, and utilization management expertise.
  • Excellent communication, interpersonal, and collaboration skills.
  • Comfort serving as the clinical authority in payor, audit, and escalation scenarios.
  • Occasional travel to meet with providers across the region

Bonus Points:

  • Knowledge of rural health systems and the unique challenges faced by rural healthcare providers and patients
  • Experience in data-driven care models, including the use of population health tools and electronic medical records (EMRs)

The base salary range for this position is $125,000 - $225,000, based on a reduced (part-time) schedule. Compensation may vary outside of this range depending on a number of factors, including a candidate’s qualifications, skills, location, competencies and experience. Base pay is one part of the Total Package that is provided to compensate and recognize employees for their work at Homeward Health. This role is eligible for an annual bonus, stock options, as well as a comprehensive benefits package.
