
Pediatric Advanced Practice Provider (APP)

We deliver 24/7 virtual first and in home medical, behavioral, and social care, working alongside families, providers, and health plans to break down barriers to quality care. The primary location for this role is fully remote. The expected schedule requirement is 4x10s, 8:00 am - 6:00 pm, Monday–Thursday or Tuesday-Friday. Active NC licensure is required.

As an APP with Imagine Pediatrics, you’ll be at the forefront of this revolutionary care model, leveraging cutting-edge technology and collaborating with a diverse interdisciplinary team to deliver high-quality, virtual-first care without the constraints of a traditional office setting.
In this role you will:
- Manage a panel of complex pediatric patients: Oversee care plans, coordinate across systems, and ensure comprehensive care delivery in a virtual-first model.
- Collaborate with the care team: Partner with pediatric RNs, navigators, dietitians, pharmacists, psychiatrists, social workers, and therapists to provide wraparound support for families.
- Empower parents and caregivers: Educate families to navigate care plans, reinforce best practices at home, and build long-term confidence in managing their child’s health.
- Deliver high-quality virtual care: Conduct comprehensive enrollment visits, acute escalations, in-home virtual assessments, sick visits, and post-hospital discharge follow-ups.
- Bridge gaps in care: Step in to address urgent needs, coordinate prescriptions, streamline referrals, and ensure continuity for families facing barriers to access.
What You Bring & How You Qualify
First and foremost, you’re passionate and committed to creating the world our sickest children deserve. You want an active role in building a diverse and values-driven culture. Things change quickly in a startup environment; you accept that and are willing to pivot quickly on priorities. In this role, you will need:
- Master’s or Doctorate degree in Nursing with current Nurse Practitioner license, or PA-C (Physician Assistant certification)
- Active CPNP-PC (Certified Pediatric Nurse Practitioner) required
- At least 4 years of NP experience; prior RN experience strongly preferred (ideally total 8+ years of clinical experience)
- Active North Carolina licensure required
- Proven experience in pediatric primary care; school-based health or medically underserved settings preferred
- Strong clinical foundation in pediatric medicine, including management of both acute and chronic conditions
- Comfortable working independently while collaborating within a multidisciplinary team
- Prior hospital or inpatient RN experience is a plus
- Experience with Medicaid, community health, or value-based care models preferred
- Virtual care experience a plus
