
Community Health Navigator, Emergency Department Follow-Up

What You’ll Do
The Community Health Navigator, Emergency Department Follow-Up provides support in assisting patients getting post-hospital care in the form of telephonic and in-person support. In this position, you will:
- Proactively engage patients post emergency department visits and provide education on ED avoidance and resources.
- Conduct virtual and telephonic visits to support patients with any in-person/community needs related to care coordination/discharge support. These could include pharmacy fills, PCP coordination, SDOH needs, and home visits.
- As part of post-discharge visits, identify any symptom worsening or concerns, and appropriately escalate to the clinical team.
- Provide education on Imagine Pediatric Services.
- Ensure patient has appointments for any needed follow-up.
- Schedule callbacks for patients with Imagine clinical teams as appropriate.
- Work in a multiplatform virtual environment as part of an interdisciplinary team.
- Coordinate with facility discharge planners and Imagine Pediatrics’ Transition of Care Nurses to ensure that there are no barriers to patients obtaining after-hospital care.
- Attend daily huddles with your team to communicate patient needs and progress.
- Liaison with hospital and other health systems to make staff aware of Imagine Pediatric program and support.
- Provide appropriate internal, social service, and/or community-based referrals.
- Perform other duties as assigned.
What You Bring & How You Qualify
First and foremost, you’re passionate and committed to reimagining pediatric health care and creating a world where every child with special health care needs gets the care and support they deserve. In this role you will need:
- Associates degree or higher
- 2+ years of relevant clinical work experience
- Emergency or inpatient settings preferred
- Experience with medically complex patient populations preferred
- Pediatrics preferred
- Have one of the following designations: Certified Community Health Worker, Certified Medical Assistant, Home Health Aide, Licensed Practical Nurse
- Knowledge of Medicaid population preferred
- Bilingual Spanish required

The hourly rate for this position ranges from $23 - 28 per hour in addition to competitive company benefits package and eligibility to participate in an employee equity purchase program (as applicable). When determining compensation, we analyze and carefully consider several factors including job-related knowledge, skills and experience. These considerations may cause your compensation to vary.
