
Clinical Pharmacy Manager

This role is remote, travel 4-6 times per year, and expected schedule requirements are Monday to Friday, 8:00am-5:00pm Eastern (preferred) or Central time.
What You’ll Do
The Clinical Pharmacy Manager provides medication management expertise and leadership for the clinical pharmacy teams embedded within the regions. This role supports implementation of medication management programs and is responsible for consistent delivery of medication management programs within the regions. The Clinical Pharmacy Manager leads the region clinical pharmacy teams and provides direct patient care alongside the teams, promoting optimal medication therapy, disease outcomes, and value of care for pediatric patients with chronic medical and behavioral health conditions. In this role you will:
- Maintain oversight of medication management services provided within the designated markets/region.
- Provide people management for the Clinical Pharmacists within the designated market/region.
- Support the Clinical Pharmacist in their people management and daily oversite of the Clinical Pharmacy Technician(s) within the designated market/region.
- Ensure training and education of the clinical pharmacy team to support consistent delivery of medication management services.
- Evaluate the quality of medication management programs and audit pharmacist intervention.
- Develop education programs for patients, specific practice-area/general education to pharmacists, providers, and other appropriate personnel as documented on an attendance form or through evaluations completed by the participant.
- Develop and iterates scalable medication management programs within areas of expertise in coordination with Director of Clinical Pharmacy and Clinical Pharmacist Specialist
- Maintain subject matter expertise of implementation, operation, and monitoring of medication management programs.
- Provide medication management services inclusive of medication access, medication education, medication adherence, comprehensive/directed medication management, and quality metric support for the designated market/region in alignment with the programs as designed by the medication management center of excellence.
- Assist with medication access management services between the patient, pharmacy, insurance, and medical provider through oversite and support of the market Clinical Pharmacy Technician.
- Provide accurate, thorough medication education, medication adherence assessment, and comprehensive/directed medication management through direct interactions with patients/caregivers, internal and external care team members, and works independently under individualized scope of clinical practice (where applicable).
- Document encounters in the patient electronic medical record, including closed-loop communication with patient care team.
- Promote the practice of evidence-based, cost-effective medication management by keeping abreast of new drugs, devices, and medical literature.
- Document/report adverse drug events (ADEs), medication allergies and medication errors per procedure.
- Liaison between the designated market/region and the medication management center of excellence.
- Practice pharmacy in compliance with all governing regulatory authorities.
- Perform other duties as assigned.

Education & Training Requirements
- Graduate of an accredited college/university of pharmacy (Pharm.D) required
- Valid license or eligible for licensure from the Board of Pharmacy in applicable state(s)
- Completion of ASHP accredited PGY-1 Pharmacy Residency required
- Completion of an ASHP accredited PGY-2 Residency (Pediatrics, Ambulatory care, health-system leadership) strongly preferred,
- Extensive advanced practice experience in an applicable clinical care setting may be considered in lieu of PGY-2 residency completion
- Applicable Board Certification (Pediatric Pharmacy, Ambulatory Care Pharmacy, or Pharmacotherapy) or eligible with successful completion within 12 months of hire
- Bilingual English/Spanish, preferred
Work Experience
- 5+ years of advanced clinical practice experience, post-PGY1 residency, in an applicable direct patient care setting required
- Previous leadership experience, such as clinical manager or residency leadership, strongly preferred
- Virtual care experience a plus
