Staff Product Manager, Care Coordination

United States (Remote)

Who We Are

Imagine Pediatrics is a tech enabled, pediatrician led medical group reimagining care for children with special health care needs. We deliver 24/7 virtual first and in home medical, behavioral, and social care, working alongside families, providers, and health plans to break down barriers to quality care. We do not replace existing care teams; we enhance them, providing an extra layer of support with compassion, creativity, and an unwavering commitment to children with medical complexity.

Families come to Imagine Pediatrics with real needs: a medication refill, a piece of medical equipment, a benefits application, follow-up after a hospital stay. As a Staff Product Manager, you’ll own the systems that make sure each of those needs is captured, has a clear owner on our care team, and actually gets delivered on. This is how we turn our care model into meaningful value for families.

AI is central to how we’ll do this at scale. You’ll decide where AI and automation can take coordination work off our care teams so they spend their time on families instead of tracking, routing and chasing work, and you’ll prove it with measured results.

Our care model blends virtual and in-home care for children with special health care needs and their families. The product you own is where care teams see what families need, pick up the work and finish it, every day.

What You'll Own

Product area: How family needs are captured, assigned, tracked and resolved across our care teams. That includes the shared view of patients and work care teams use every day; tasking and handoffs between roles; messaging with families by chat, text and email; internal care team communication; and follow-up after hospital discharges.

AI and automation in care coordination to:

  • capture needs from family conversations and messages
  • route and assign work to the right person
  • prioritize what’s most urgent
  • summarize a need’s history for whoever picks it up next
  • flag needs that are stalling before a family feels it

You’ll set the bar for where AI belongs, where it doesn’t, and how we measure whether it’s working.

  • Primary users: Navigators, nurses, resource specialists, pharmacists and other care team members coordinating care. Families are users too, through the messages and updates they receive.
  • North star: % of family needs resolved within target time. You’ll also track how quickly each need gets a named owner, how many needs stall without progress, and how much care team time goes to coordination overhead rather than to families.
  • First-year outcomes:
    • Every family need has a named owner and a visible status, and care teams stop working across fragmented tools.
    • One shared view of the patients and work each care team owns, prioritized by what’s most urgent for families.
    • AI-assisted capture, routing or prioritization in production, with a measured reduction in coordination time and no drop in quality.

Other teams depend on what you build. Outreach routes newly enrolled families into your care team views, and the family app shows families the status of their needs. Your decisions shape their roadmaps.

What You'll Do

Set direction in ambiguous problem spaces

  • Define the multi-quarter strategy for how family needs are captured, owned, worked and resolved across care teams.
  • Frame problems by synthesizing input from clinical leaders, care teams, operations and data, often before anyone has named the problem.
  • Turn open-ended operational problems into a clear strategy, and make decisions with incomplete information when waiting would cost families.

Challenge how the work gets done

  • Push stakeholders to examine their operational workflows: why each step exists, what can be simplified or automated, and what won’t scale as we grow.
  • Don’t digitize a broken process; redesign the workflow first, then build for it.
  • Say no, with clear reasoning, to requests that don’t move outcomes, and hold that boundary with senior stakeholders.

Lead with AI where it creates real leverage

  • Identify the coordination work best suited to AI and automation, and the places where human judgment has to stay in the loop.
  • Partner with engineering and data to take AI features from prototype to production, with clear evaluation criteria, safety guardrails and care team trust.
  • Measure AI by the outcomes it moves (time saved, needs resolved faster, fewer dropped handoffs), not by what launched.

Drive discovery and execution at scale

  • Own the full product lifecycle for your area using a build–measure–learn approach.
  • Lead discovery directly with care teams to understand real workflows, clinical realities and operational constraints.
  • Sequence migrations off legacy tools, make hard scope tradeoffs and ship improvements that compound.

Lead cross-functionally without authority

  • Partner with engineering, clinical leaders, care teams, data and operations to design workflows that hold up in the real world.
  • Broker tradeoffs between product, clinical workflows and operational capacity with senior stakeholders.
  • Define how other teams plug into care coordination (how work reaches care teams and how families see progress on their needs), and influence their roadmaps where they depend on you.

Own outcomes, not output

  • Own the success metrics for care coordination and hold the org accountable to them, for both family outcomes and care team efficiency.
  • Drive adoption with care teams, not just launches: a system only works if teams actually work from it.
  • Kill or rework what isn’t moving the numbers.

Raise the bar

  • Mentor other PMs and model strong discovery, writing, prioritization and decision-making.
  • Become a recognized internal expert in pediatric care coordination and AI-enabled care operations.

What You Bring & How You Qualify

First and foremost, you’re motivated by improving the health and lives of children with special health care needs and their families, and you measure your success by the impact you have on them. You’re comfortable in ambiguity and bring clarity, judgment and momentum to problems others find intractable. You ask hard questions about how work gets done, you’re not afraid to say no, and you hold your ground when the outcome is at stake.

You likely bring:

  • 8+ years of product management experience, ideally in healthcare or care delivery
  • A track record of owning a workflow-heavy product area from strategy through launch and adoption
  • Shipped AI or automation features for operational users, with measured impact on efficiency or outcomes
  • Sound judgment about AI’s limits: evaluation, failure modes, and when a human needs to stay in the loop
  • Experience building tasking, case management, worklist or messaging systems
  • Experience redesigning operational workflows for scale, not just building tools on top of them
  • Examples of pushing back on senior stakeholders and holding a boundary to protect outcomes
  • Experience leading migrations off legacy systems without disrupting the teams that depend on them
  • Strong data-modeling instincts: you can reason about entities, ownership and state with engineers
  • Comfort in fast-moving startup environments where priorities evolve
  • Experience mentoring PMs
  • Excellent communication with technical and non-technical audiences, including executives

Bonus if you have experience with:

  • Care management platforms or EHR-integrated workflows
  • Medicaid populations or value-based care
  • LLM-based products in regulated or clinical setting
