
Program Manager, Provider Operations

As a Program Manager within Interwell Health's Provider Operations Team, your key responsibility will be to act as a liaison between our provider network and nephrology practices, facilitating a smooth transition to Value-Based Care. This role requires a diligent work ethic, excellent communication skills, and the ability to work independently and collaboratively with external partners and internal, cross-functional teams.
The work you will do:
- Bring deep understanding of the objectives and complexities of intricate Value-Based Care Programs and/or agreements, such as:
- CMS Kidney Care Contracting
- IWH Payor Agreements
- IWH Practice Participation Agreements
- Serve as primary contact and subject matter expert for IWH Network practices.
- Assist our nephrology partners in achieving success within value-based care by uncovering valuable insights within data and identifying opportunities for improvement.
- Execute company initiatives, ensuring the timely completion of deliverables.
- Maintain accurate documentation of provider interactions/visits.
- Deliver timely training and/or onboarding to Network Providers.
- Verify that information presented to providers is both accurate and timely
The skills and qualifications you will need:
- Bachelor’s degree required; Advanced Degree desirable
- Proven experience working with physicians and practices, nephrology-specific experience a plus
- Superior attention to detail and exceptional follow-through
- Should possess an independent and proactive approach, showcasing dedication to continuous learning
- Excellent communication and interpersonal skills to effectively collaborate with providers, stakeholders, and internal teams.
- Must be willing to travel up to 50% of the time to practices (assigned regions will be discussed during the interview process)
