The Registered Dietitian plays a key role in our interdisciplinary care team, partnering with patients to manage kidney disease through personalized, virtual nutrition support. In this role, you’ll assess nutritional needs, develop meaningful care plans, and coach patients through sustainable dietary and lifestyle changes that improve health outcomes. This is an opportunity for a renal nutrition professional who is passionate about chronic disease management and motivated by delivering patient‑centered care in a value‑based, outcomes‑driven environment.
Note: We are hiring for multiple openings, with preference for candidates who reside in and hold licensure in select states; details will be discussed during the interview process.
What You’ll Do:
- Collaborate with interdisciplinary team members, including nurses, social workers, care coordinators, and others, to deliver comprehensive virtual care services to patients.
- Conduct virtual nutritional assessments for patients with chronic kidney disease (CKD) and other co-morbid conditions, utilizing available patient information, self-reported health data, and dietary intake to inform care recommendations.
- Develop personalized, evidence-based nutrition care plans and calculate nutrition requirements tailored to the needs and preferences of patients at various CKD stages, often with other comorbid dietary considerations, focusing on dietary and lifestyle modifications that support holistic disease management.
- Educate patients and their caregivers on kidney-related dietary and lifestyle adjustments, including nutrition components related to kidney disease, co-morbidities, and conditions.
- Provide patients with resources and tools to manage their health conditions.
- Maintain thorough and timely documentation of all patient interactions, care plans, and interventions, ensuring compliance with company policies and regulatory requirements.
- Practice within the registered dietitian scope of practice and make necessary interdisciplinary referrals to nurses, social workers, care coordinators, and physicians when applicable.
What You’ll Need:
- Bachelor’s Degree in Nutrition, Dietetics, or a related field from an accredited institution
- Must hold an active unrestricted Registered Dietitian certification issued by the Commission on Dietetic Registration
- Minimum of relevant state dietitian licensure or registration; willingness to obtain and maintain additional state dietitian licensures as needed
- 2+ years’ experience in the delivery of care to patients with kidney disease or other chronic diseases
- Knowledge of healthcare regulations (including HIPAA) and a strong commitment to patient confidentiality, ethics, and professional standards.
- Proficient in using virtual care platforms to deliver education, document care, collaborate with interdisciplinary teams, and create a positive, engaging, and impactful patient experience remotely.
- Proficiency in patient counseling and education, including assessing understanding, navigating sensitive conversations, and addressing health concerns.
- Ability to adapt care approaches and problem‑solve effectively to meet patients’ changing needs in a virtual or remote setting.
- Experience developing individualized nutrition guidance that supports patient self‑management, prevention, and sustainable behavior change.
- Strong communication skills, with the ability to translate complex nutrition concepts into clear, practical, and empathetic guidance for patients and caregivers.
