Position Summary

We’re looking for a proactive, analytical, and operations-savvy Solutions Delivery Associate to support the ongoing delivery and growth of our Chronic Care Management (CCM) solution for existing customers. This role is central to ensuring care team productivity, accurate performance reporting, and efficient day-to-day operations across internal and client teams.

You’ll work closely with the Solutions Delivery Manager, implementation teams, product, and business intelligence, as well as client stakeholders. Over time, this role is expected to grow into a client-facing delivery lead, managing your own portfolio of CCM clients.

Key Responsibilities

Operational Support & Coordination

  • Support the Delivery Manager in managing the day-to-day operations of our CCM programs.
  • Help track care manager productivity, caseload balance, and workload distribution.
  • Assist in planning and tracking operational KPIs related to care delivery, program adherence, and client performance.
  • Act as a central point of coordination between the delivery, implementation, and data teams to ensure smooth execution of client programs.

Reporting & Data Management

  • Develop and maintain recurring and ad hoc reports for clients and internal stakeholders.
  • Conduct patient eligibility analyses and assist in identifying opportunities for patient enrollment and outreach.
  • Partner with the data team to ensure accurate data sharing, validation, and insights reporting.
  • Analyze trends in program performance and prepare executive summaries and visualizations to support client discussions.

Client Collaboration & Communication

  • Participate in client calls to support discussions around program performance, data accuracy, and improvement initiatives.
  • Document client-specific workflows, configurations, and feedback to inform solution adjustments.
  • Help ensure that clients receive timely, accurate data and reports to support their CCM program success.

Team & Cross-Functional Partnership

  • Collaborate with the CCM Solution Manager to align reporting, workflows, and service delivery with solution best practices.
  • Support the implementation team during onboarding and handoff to delivery, ensuring a smooth transition.
  • Work across cross-functional teams to triage and resolve delivery issues.

Growth & Leadership Opportunity

  • Gain exposure to client management with the opportunity to take on your own portfolio of clients over time.
  • Contribute to the continuous improvement of delivery tools, processes, and reporting templates.
    Develop a deep understanding of our CCM solution, client needs, and the healthcare regulatory environment.

Qualifications

  • Bachelor’s degree in Operations, Analytics
  • 1–3 years of experience in operations, data analysis, or customer support—ideally in a tech-enabled or services-based setting.
  • Strong Excel/Google Sheets skills (pivot tables, lookups, charts).
  • Excellent organization, communication, and time management skills.
  • Ability to work collaboratively across teams while managing competing priorities.
  • A growth mindset, with the ability to learn quickly and a desire to eventually own client relationships.

Preferred (but not required):

  • Familiarity with healthcare data, workflows, and terminology (especially related to Chronic Care Management, population health, or value-based care)
  • Experience with BI tools (Tableau, Looker, Power BI)
