Contract Physical Therapist - Home Care

Contract position. 3 months with possibility of extension

Overview:

We’re seeking a compassionate and skilled Contract Physical Therapist to provide in-home care for a large visiting nurse program in Boston. Join a team that values autonomy, flexibility, and exceptional patient-centered care. All Boston neighborhoods are served.

Requirements:

  • Graduate of a physical therapy program.
  • Valid Massachusetts physical therapy license in good standing.
  • Valid CPR certification.
  • Driver’s license and reliable means of transportation to conduct home visits.
  • Work authorization in the US

Experience & Qualifications:

  • Interest in working with the geriatric and/or a medically complex population.
  • Willingness and interest in traveling to patients’ homes.
  • Prior therapy or rehabilitation experience a plus.
  • Open to serving medical needs of a multicultural and diverse patient population.
  • Bilingual language skills helpful.

Responsibilities:

  • Deliver evidence-based physical therapy to patients in their homes
  • Develop and implement individualized care plans in coordination with referring physicians
  • Educate patients and caregivers on therapeutic exercises and mobility techniques
  • Monitor progress and adjust treatment plans as needed
  • Collaborate with interdisciplinary team members and case managers
  • Document all care activities in accordance with state and agency standards
  • Participate in care team meetings and continuous quality improvement efforts
