Job Title: Clinical Assessment RN
Location: Hybrid (in-person home visits & telephonic visits): Multiple locations and positions available across Eastern, Central, and Western Massachusetts!
Employment Type: Full-Time
Rate: $65/hr
Start Date: ASAP
Shifts: Monday–Friday. 12 assessments per week, no weekends
Contract Length: 3-6 months with the ability to extend (Full-Time Equivalent, 40 hrs/week)
About the Role
LCG is partnering with Commonwealth Care Alliance (CCA) and is seeking a dedicated and compassionate Clinical Assessment RN to join our Assessment Unit. This position plays a critical role in identifying the clinical and social needs of CCA members through in-person assessments and ensuring timely, high-quality documentation that supports individualized care plans.
Working in a geographically assigned territory, you will conduct comprehensive, face-to-face assessments using the Minimum Data Set (MDS) and supplemental tools, helping to identify unmet needs and enable the delivery of person-centered care. Key Responsibilities
- Conduct in-person (and when appropriate, telephonic or virtual) clinical assessments using MDS and screening tools such as PHQ-9, EASI, AUDIT, and DAST.
- Use clinical judgment to escalate urgent member needs to supervisory staff as necessary.
- Document assessments and interactions accurately using CCA’s electronic health records and care management platforms.
- Maintain an organized assessment schedule and track completion timelines.
- Ensure clear, effective communication with care team members to support coordinated care delivery.
- Provide telehealth-based assessments using video or chat, when clinically appropriate.
- Maintain patient confidentiality and comply with all organizational policies and procedures.
Required Qualifications
- Associate Degree in Nursing
- Active RN license in good standing in the Commonwealth of Massachusetts
- At least 1 year of clinical experience, including clinical rotations
- Ability to travel across Massachusetts and conduct home visits (driver’s license required)
- Proficiency in English
Preferred Qualifications
- Bachelor’s Degree in Nursing (BSN)
- Experience with electronic medical records (EMR)
- Previous experience in care management or community-based care
- Bilingual in English and a second language (e.g., Spanish, Portuguese, Vietnamese, Russian)
