Social Worker - Older Adults Team
Liquid Personnel is recruiting a Social Worker to join our client’s Older Adults Team based in Merton. In this role, you will deliver high-quality, person-centred social work support to older adults with complex needs. Your focus will be on promoting independence, safeguarding wellbeing, and ensuring full legal compliance under the Care Act 2014 and other relevant legislation.
What will your responsibilities be?
- Undertake holistic assessments of need, risk, and capacity.
- Develop and review personalised care and support plans.
- Carry out Mental Capacity Act assessments and Best Interests decisions.
- Identify and respond to safeguarding concerns.
- Promote the rights, dignity, and autonomy of older people.
- Collaborate with health professionals, housing teams, voluntary organisations, and care providers.
- Maintain accurate and timely case records.
- Ensure compliance with statutory duties and local policies.
- Act as an advocate, ensuring older adults’ voices are heard in decision-making.

Qualifications & Experience:
To be successful in this role you must have,
- Social Work England registration
- Eligible to work in the UK
- Hold a full UK driving licence
- Post-qualified experience
- Degree level or equivalent in Social Work
