Job Title: Social Worker - Countywide Reviewing
Location: East Riding of Yorkshire (Hybrid)
Pay Rate: £35 per hour
Job Description:
What will your responsibilities be?
In this role, you will:
- Conduct proactive reviews of care arrangements: Assess residents in care homes with 1:1 support and individuals receiving Direct Payments to ensure packages are appropriate, least restrictive, and proportionate.
- Update assessments and care/support plans: Visit individuals in their home environment, review their needs, and revise documentation to reflect current circumstances.
- Submit funding requests and liaise with providers: Work collaboratively with care providers to adjust support levels, reduce unnecessary restrictions, and ensure funding applications are accurate and timely.

Qualifications & Experience:
To be successful in this role you must have:
- Social Work England registration
- Eligible to work in the UK
- Hold a full UK driving licence
- Post-qualified experience
- Degree level or equivalent in Social Work
