Job Title: Social Worker - Preparing for Adulthood - Learning Disabilities & Autism
Location: Rochdale
Pay Rate: £35 per hour 

Liquid Personnel is recruiting an Adult Social Worker to join its client’s Preparing for Adulthood - Learning Disabilities & Autism Team, based in Rochdale.

What will your responsibilities be? 

  • Conducting Care Act assessments: Evaluate young people (usually 17+) to determine eligibility for adult social care services.
  • Coordinating transition planning: Work with schools, families, and professionals to ensure smooth movement from children’s to adult services.
  • Supporting independence and community inclusion: Help individuals develop daily living skills, access housing options, and engage in employment or training pathways.

Qualifications and Experience: 

To be successful in this role you must have.  

  • Social Work England registration 
  • Eligible to work in the UK 
  • Hold a full UK driving licence 
  • Post-qualified experience 
  • Degree level or equivalent in Social Work
