Occupational Therapist
Liquid Personnel is recruiting an Occupational Therapist at based in Hull.

About This Role:
As part of Hull’s integrated Adult Social Care service, you’ll play a key role in helping people stay independent, resilient and safe in their own homes. You’ll work closely with social care colleagues, health professionals, housing partners and community organisations to deliver meaningful, solution‑focused interventions.
Your work will empower people to identify their own goals, build on their strengths and maintain control over their wellbeing.
What will your responsibilities be?
- Deliver strengths‑based, person‑centred OT assessments that support people to shape their own lives and maintain independence.
- Provide practical support including equipment fitting, demonstrations, adaptations and maximising the use of assistive technology.
- Offer information, advice and guidance to help people:
- Build self‑care skills
- Strengthen support networks
- Access community groups and voluntary organisations
- Use universal services wherever possible
- Support people to use Connect to Support for information, advice and self‑funded services.
- Help individuals build community connections that reduce isolation, prevent hospital admissions and support timely discharge.
- Refer into and work alongside the Social Prescribing Service where beneficial.
- Lead or contribute to case management, including case conferences, safeguarding reviews and best‑interest decisions.
- Work collaboratively across Adult Social Care, providing advice and training to colleagues to support positive outcomes.
- Work jointly with housing partners and surveyors to ensure adaptations meet assessed needs, including joint assessments and reviews.

Qualifications and Experience:
To be successful in this role you must have;
- A strengths‑based, solution‑focused practitioner who values people as experts in their own lives.
- Strong assessment skills and confidence delivering proportionate, person‑centred interventions.
- Experience with equipment, adaptations and assistive technology.
- Ability to build relationships with individuals, families, communities and partner agencies.
- HCPC‑registered Occupational Therapist.
