Senior Occupational Therapist – Equipment and Adaptation

Liquid Personnel is seeking a Senior Occupational Therapist to join their Equipment & Adaptations Team on a full‑time locum basis in Wolverhampton. This role covers work with both adults and children, providing specialist OT assessments, equipment prescriptions, and recommendations for both minor and major adaptations.

This is a great opportunity for an experienced fieldwork OT confident with complex cases and working across diverse client groups.

What will your responsibilities be? 

  • Complete holistic functional assessments for adults and children.
  • Recommend and prescribe equipment, minor adaptations, and contribute to major adaptation planning.
  • Work as part of the Equipment & Adaptations Team, handling a varied, busy caseload.
  • Provide clear, defensible clinical reasoning and strong risk‑management decisions.
  • Liaise with housing teams, contractors, social workers, health partners, and families.
  • Maintain accurate case notes using Eclipse or similar social care recording systems.
  • Support service users to maximise independence, safety, and quality of life.
  • Manage fieldwork responsibilities independently while contributing to the wider team.

Qualifications & Experience: 

  • Degree / Diploma in Occupational Therapy.
  • Strong experience in equipment provision, minor/major adaptations, and community fieldwork OT practice.
  • Experience working with both adults and children.
  • Confident using Eclipse or another social care case‑management system.
  • Ability to start immediately (essential).
  • Strong decision‑making skills and the ability to work independently.
