Occupational Therapist – Hospital Discharge and Reablement

Our client is seeking an experienced and resilient Occupational Therapist to support increased service demand following the successful rollout of the Home First contract based in Wiltshire. This role is ideal for an OT who thrives in fast‑paced, acute discharge environments and enjoys collaborative working across health and social care.

About the Role

You will work in close partnership with staff across both local acute hospital trusts — Great Western Hospital (GWH) and Royal United Hospital (RUH) — to support safe, timely and effective hospital discharge into Wiltshire’s Reablement Service.

This is a dynamic, high‑demand role requiring strong clinical reasoning, excellent organisational skills and confidence managing rapidly changing situations.

Key Responsibilities

  • Support hospital discharge planning for adults moving into the Reablement Service.
  • Work collaboratively with acute hospital colleagues at GWH and RUH.
  • Carry out community‑based assessments and functional evaluations.
  • Produce high‑quality reports, recommendations and care plans.
  • Manage a varied caseload within a fast‑paced environment.
  • Ensure timely, safe, person‑centred discharge pathways are achieved.
  • Contribute to a multidisciplinary approach across health and social care.

Working Pattern & Hybrid Setup

  • 37 hours per week
  • 50% of admin time must be completed in the office (County Hall, Trowbridge)
  • Remaining time split between WFH and community visits
    • Example: If 3 days are spent on community visits, you would then complete 1 day in the office + 1 day working from home

  • 3‑month assignment, with strong potential for extension
  • Council does not pay accommodation or travel expenses for those living outside Wiltshire
