Social Care Occupational Therapist

Liquid Personnel is seeking an experienced Social Care Occupational Therapist to join a supportive and transformative Adult Social Care team in Kirklees. This role forms part of an exciting 12‑month transformation project, offering flexibility, autonomy, and the chance to make a real impact within the community.

You will play a key role in delivering high‑quality functional assessments, promoting independence, and supporting residents through strengths‑based, outcome‑focused practice.

What will your responsibilities be? 

• Carry out Care Act assessments to identify needs and promote independence.
• Complete assessments and recommendations under Disabled Facilities Grant (DFG) legislation.
• Provide professional guidance on equipment, adaptations, and environmental modifications.
• Work collaboratively within a supportive multidisciplinary team.
• Maintain accurate and compliant documentation in line with statutory requirements.
