Social Worker - Community Mental Health

Liquid Personnel is recruiting Social Worker for its client's Community Mental Health Team at Dorset.
Our client is seeking an experienced and compassionate full-time Social Worker to join our Community Mental Health Team (CMHT) in Weymouth and Portland. This vital role offers the opportunity to support individuals with complex mental health needs through comprehensive assessments, care planning, and community-based interventions. Our successful candidate will have a strong background in mental health social work, excellent communication skills, and the ability to work collaboratively within a multidisciplinary team. What will your responsibilities be?

  • Conduct Care Act assessments, develop individualized support plans, and perform timely reviews.
  • Complete S117 assessments and reviews for eligible individuals.
  • Assess a wide range of complex mental health patients both clinically and within community settings.
  • Conduct robust risk assessments and manage cases involving MARMS, MAPPA, DoLS, and Court of Protection matters.
  • Collaborate closely with the multidisciplinary team including Community Psychiatric Nurses, psychiatrists, psychologists, and other community professionals. Maintain accurate and detailed documentation to support high-quality care and compliance with statutory requirements.
  • Engage in community visits to monitor and support service users effectively.
  • Contribute to a culture of equality, diversity, and inclusion, ensuring services are accessible and sensitive to all individuals’ backgrounds and needs.

Qualifications & Experience:

To be successful in this role you must have, 

  • Professional Qualification: A recognised professional qualification in social work, such as an MA, BA, BSc in Social Work, Diploma in Social Work, or CQSW (Certificate of Qualification in Social Work).
  • Registration: Must be registered with Social Work England, the regulatory body for social workers in England.
  • Experience: 2+ years post qualified experience as an adult social worker in a statutory setting. 
  • Knowledge of Legislation: Thorough understanding of relevant legislation, such as the Care Act 2014 and the Mental Capacity Act 2005.
  • Skills: Strong assessment and analysis skills, as well as the ability to work effectively within a multidisciplinary team.
  • Driving Licence: A full UK driving licence and access to a car are often required for client visits.
