Social Worker - Home First Team (D2A)

Liquid Personnel is recruiting an Adult Social Worker to join its client’s Home First team, based in Dorset.

The primary focus of this role is to deliver a Home First / Discharge to Assess approach, supporting people to return home from hospital safely and in a timely manner. You will be a key contributor to a strong partnership between health, social care, and the Home, Independence, and Crisis Support (HICS) team.

Previous experience in hospital discharge is desirable. You will be expected to demonstrate a strong knowledge and understanding of relevant legislation, strengths‑based and person‑centred practice, and experience working with complexity, including the Court of Protection.

The role involves both community and hospital visits across Dorset and requires office attendance for a minimum of two days per week.

What will your responsibilities be?

In this role, you will:

  • Ability and need to be able to attend all our acute and community hospitals across Dorset to support and facilitate discharge.
  • Requirement to come into the office to support duty once a week
  • Hospital Discharge Planning: Lead on social work assessments for complex hospital discharges, ensuring timely and effective planning in line with national and local guidance (e.g., Hospital Discharge Service Policy and Operating Model).
  • Strengths-Based Assessment: Undertake comprehensive social care assessments under the Care Act 2014, identifying individuals' strengths, assets, and desired outcomes.
  • Risk Management: Conduct robust risk assessments to ensure safe discharge and ongoing safety in the community, escalating concerns appropriately.
  • Care Planning & Coordination: Develop, implement, and review personalised care and support plans, coordinating a range of services including reablement, intermediate care, community equipment, adaptations, and long-term care where necessary.
  • Multi-Agency Working: Work collaboratively and proactively with NHS colleagues (ward staff, therapists, discharge coordinators), reablement teams, GPs, housing providers, voluntary sector organisations, and other agencies to ensure seamless transitions of care.
  • Safeguarding: Identify and respond to safeguarding concerns in line with Dorset Council's policies and procedures, ensuring the safety and well-being of vulnerable adults.
  • Mental Capacity Act (MCA) & Best Interests: Apply the principles of the Mental Capacity Act 2005, undertaking mental capacity assessments and making Best Interests decisions where required, in partnership with individuals and their representatives.
  • Duty & Prioritisation: Manage a busy caseload effectively, prioritising work to meet urgent discharge demands and deadlines.

Qualifications & Experience:

To be successful in this role you must have:

  • Professional Qualification: A recognised professional qualification in social work, such as an MA, BA, BSc in Social Work, Diploma in Social Work, or CQSW (Certificate of Qualification in Social Work).
  • Registration: Must be registered with Social Work England, the regulatory body for social workers in England.
  • Experience: 2+ years post qualified experience as an adult social worker in a statutory setting. 
  • Knowledge of Legislation: Thorough understanding of relevant legislation, such as the Care Act 2014 and the Mental Capacity Act 2005.
  • Skills: Strong assessment and analysis skills, as well as the ability to work effectively within a multidisciplinary team.
  • Driving Licence: A full UK driving licence and access to a car are often required for client visits.
