Social Worker – Intermediate Care
Our Client’s Intermediate Care Service is seeking a dedicated and motivated Social Worker to join their fantastic team in Southwark London Borough.
This 6‑month fixed‑term opportunity has been created to support the service in addressing a waiting list backlog, ensuring residents receive timely, high‑quality support.
Working as part of an integrated Health and Social Care team, you will play a key role in supporting residents through their recovery and reablement journey. You will work closely with multi‑disciplinary colleagues to deliver coordinated, person‑centred care that promotes independence, wellbeing, and positive outcomes.
Key Responsibilities
- Completing Care Act assessments, reviews, and personalised care and support plans
- Carrying out Mental Capacity Assessments and contributing to Best Interests decision‑making
- Managing safeguarding concerns, assessing risk, and embedding effective protection plans
- Working collaboratively with health partners, community resources, and informal networks to deliver holistic, strengths‑based support
- Ensuring timely, accurate case recording and contributing to a culture of learning and reflective practice
About You
You will be a registered Social Worker who is passionate about empowering people and promoting independence. You will bring a strengths‑based, person‑centred approach and be confident working within a multi‑disciplinary environment.
You will be able to demonstrate:
- Experience in integrated health and social care, reablement, or rehabilitation settings
- Strong knowledge of social care legislation, policy, and safeguarding practice
- A commitment to equality, diversity, and inclusion
- Excellent communication, assessment, and organisational skills
- The ability to manage competing demands while keeping residents at the centre of decision‑making

Intermediate Care is an innovative, inclusive, and supportive service operating across five local areas within the borough. Working in small multi‑disciplinary groups enables the team to build strong community links, understand local needs, and respond effectively to diverse communities.
