Senior Occupational Therapist – Reablement at Home
Liquid Personnel is recruiting a Senior Occupational Therapist to join its client’s Reablement Team, based within the London Borough of Hackney.
This Senior OT role supports Hackney’s continued shift towards a reablement‑first approach, promoting independence, reducing long‑term care dependency, and improving outcomes for local residents.
The Reablement at Home service is a short‑term, therapy‑led intervention (typically lasting up to six weeks), supporting individuals to regain skills and confidence following hospital discharge.
What will your responsibilities be?
In this role, you will:
- Deliver therapy‑led reablement interventions
- Lead on person‑centred, goal‑focused reablement plans
- Support hospital discharge pathways
- Promote a “doing with” rather than “doing for” approach
- Enable independence, autonomy and wellbeing
- Work collaboratively with health, social care and community partners
Qualifications & Experience:
To be successful in this role, you must have:
- HCPC‑registered Occupational Therapist
- Strong experience in reablement or discharge pathways
- Ability to work autonomously and lead best practice
