Social Worker - Hospital Discharge Team
Liquid Personnel is seeking an experienced Social Worker to join its clients Hospital Discharge Team, supporting citizens across discharge pathways.
What will your responsibilities be?
- Assess and support citizens on discharge from hospital across three pathways
- Complete post-discharge assessments and reviews
- Undertake Care Act Assessments (CAA) and support planning
- Carry out Best Interest (BI) decisions and Mental Capacity Act (MCA) work
- Lead on safeguarding enquiries and interventions
- Provide duty cover as required
- Attend Quality Assurance panels for funding provisions
- Support Discharge to Assess (D2A) pathways
Requirements:
- Proven experience within hospital discharge settings
- Strong working knowledge of Care Act, MCA, safeguarding, and discharge processes
- Ability to manage a fast-paced caseload and work under pressure
- Excellent assessment and decision-making skills
- Available to start immediately
Qualifications & Experience:
To be successful in this role you must have,
- Social Work England registration
- Eligible to work in the UK
- Hold a full UK driving licence
- Post-qualified experience
- Degree level or equivalent in Social Work
