Advanced Practitioner – Intermediate Care

Liquid Personnel is recruiting an experienced Advanced Practitioner to join its client’s Intermediate Care service based in Somerset. This is a full‑time agency role supporting complex casework, hospital discharge pathways, and admission avoidance within a multi‑disciplinary environment.

What will your responsibilities be?

  • Lead on complex assessments, including Care Act assessments, discharge planning and risk assessments
  • Support timely hospital discharges and admission avoidance through effective intervention planning
  • Manage a complex caseload, providing expert guidance on high-risk and high-need cases
  • Develop and review person-centred care and support plans that promote independence and reablement
  • Undertake safeguarding enquiries and ensure risk is managed appropriately
  • Apply the Mental Capacity Act, including Best Interests decision-making
  • Provide professional support, mentoring and guidance to less experienced staff
  • Work closely with acute hospitals, community health teams and wider multi-agency partners
  • Prepare clear, high-quality reports and maintain accurate case records
  • Support service development and contribute to best practice within the team

Qualifications & Experience:

To be successful in this role you must have:

  • Social Work England registration
  • Eligible to work in the UK
  • Degree level or equivalent in Social Work
  • Significant post‑qualified experience within Adult Social Care
  • Experience within Intermediate Care, hospital discharge, or community settings
  • Experience managing complex caseloads and high-risk cases
  • Experience working within multi-disciplinary and multi-agency environments
  • Experience providing supervision, mentoring, or practice leadership
  • Strong knowledge of the Care Act 2014, Mental Capacity Act 2005, and Human Rights Act
  • Excellent assessment, analytical, communication, and report-writing skills
  • Ability to manage competing priorities and complex workloads
  • Competent IT skills for case management systems
  • Full UK driving licence and access to a vehicle (essential)
