Social Worker – Care Assessment and Triage
Liquid Personnel is recruiting an experienced Social Worker to join their Adult Care Assessment and Triage Team.
This frontline role focuses on triage and assessment at the “front door” of Adult Social Care, supporting adults with a wide range of needs, disabilities, and circumstances. You will complete strengths-based, person-centred assessments within short timescales, often working with varied and sometimes complex or unusual casework.
What will your responsibilities be?
- Undertaking timely triage and assessment of Adult Social Care referrals at the front door service.
- Managing a varied caseload, responding promptly to individuals with a wide range of needs, disabilities, and presenting circumstances.
- Carrying out strengths-based, person-centred assessments using remote communication tools (telephone, online platforms).
- Applying Care Act 2014 principles to determine eligibility and appropriate outcomes.
- Providing early intervention, advice, guidance, and signposting to appropriate services.
- Supporting quick and effective outcomes through proportionate assessments and decision-making.
- Identifying safeguarding concerns and contributing to safeguarding processes, ensuring Making Safeguarding Personal principles are applied.
- Undertaking Mental Capacity Assessments where required and contributing to Best Interests decisions.
- Working within key legislative frameworks including:
- Care Act 2014
- Mental Capacity Act 2005 (MCA)
- Human Rights Act 1998 (HRA)
- Collaborating closely with multi-disciplinary teams, including health professionals, internal services, and partner agencies.
- Working autonomously while maintaining strong communication within the wider team.
- Maintaining accurate, timely, and high-quality records in line with statutory requirements.
- Demonstrating professional curiosity, sound judgement, and a proactive, solution-focused approach.
Qualifications and Experience:
To be successful in this role you must have;
Professional Qualification:
- MA, BA, BSc in Social Work, Diploma in Social Work, or CQSW
Registration:
- Must be registered with Social Work England
Experience:
- Minimum 2 years’ post-qualified experience, including completion of ASYE
- Strong background within Adult Social Care
- Experience in assessment, triage, or frontline duty work is highly desirable
- Experience in strengths-based practice and person-centred approaches
- Experience undertaking Care Act assessments and applying eligibility criteria
- Experience in safeguarding and multi-agency working
- Experience supporting individuals through early intervention and signposting
Legislation Knowledge:
Strong working knowledge of:
- Care Act 2014
- Mental Capacity Act 2005 (MCA)
- Human Rights Act 1998 (HRA)
- Safeguarding Adults legislation, including Making Safeguarding Personal
Other Requirements:
- Full UK driving licence and access to a car is desirable
Skills:
- Strong assessment and analytical skills in fast-paced environments
- Ability to manage varied caseloads with a focus on timely outcomes
- Excellent communication skills, particularly for remote assessment work
- Ability to work both autonomously and collaboratively within a team
- Confidence in decision-making and prioritisation under pressure
- High standard of report writing and case recording
- Strong organisational skills and ability to manage competing demands
- Resilience and adaptability when working with diverse and complex cases
- Proactive, positive, and “can-do” attitude
