Adult Social Worker – Transfer of Care

Our client in Cambridgeshire is seeking an experienced Adult Social Worker to join their Transfer of Care Team, supporting adults transitioning from hospital and rehabilitation settings back into the community.
This is a fast-paced role focused on hospital discharge, post-discharge support, and safeguarding, where you will deliver person-centred, strengths-based interventions to achieve safe and effective outcomes.
You will work collaboratively within a multi-disciplinary team, completing assessments, managing risk, and promoting independence for individuals following discharge, both in hospital and community settings.

Key Responsibilities

Core duties include:

  • Managing a caseload of adults requiring hospital discharge and transfer of care support.
  • Undertaking Care Act 2014 assessments, discharge reviews, and strengths-based support planning.
  • Completing Mental Capacity Assessments and contributing to Best Interests decisions where required.
  • Leading and contributing to Section 42 safeguarding enquiries and wider safeguarding processes.
  • Supporting safe and timely discharges from hospital and rehabilitation services into the community.
  • Working closely with individuals, carers, and families to ensure person-centred outcomes are achieved.
  • Promoting independence through reablement, community services, and informal support networks.
  • Assessing and managing risk, ensuring proportionate, defensible decision-making.
  • Working collaboratively within multi-disciplinary teams (MDT) including health professionals, discharge coordinators, and housing partners.
  • Supporting individuals post-discharge in community settings, including care homes and private residences.
  • Maintaining accurate, timely, and high-quality case records in line with statutory requirements.
  • Applying a strengths-based and outcome-focused approach to all interventions.
  • Operating effectively within a fast-paced environment, prioritising urgent work and managing competing demands.

Qualifications & Experience

Professional Qualification:

  • MA, BA, BSc in Social Work, Diploma in Social Work, or CQSW

Registration:

  • Must be registered with Social Work England

Experience:

  • Minimum 2 years’ post-qualified experience, including completion of ASYE
  • Strong background within Adult Social Care
  • Essential hospital experience within discharge or acute settings
  • Experience working in a fast-paced environment
  • Experience completing:
    • Care Act assessments and discharge planning
    • Mental Capacity Assessments
    • Safeguarding enquiries (including Section 42)
  • Experience of multi-disciplinary working
  • Experience supporting individuals transitioning into community-based care

Legislation Knowledge

Strong working knowledge of:

  • Care Act 2014
  • Mental Capacity Act 2005 (MCA)
  • Human Rights Act 1998 (HRA)
  • Safeguarding Adults legislation (including Section 42)

Skills

  • Strong assessment and analytical skills in complex discharge situations
  • Ability to manage a busy caseload in a fast-paced environment
  • Excellent communication and interpersonal skills
  • Ability to work effectively within multi-agency and MDT settings
  • High standard of report writing and case recording
  • Strong organisational skills and ability to prioritise workload
  • Ability to balance risk, independence, and safeguarding
  • Resilient, proactive, and solution-focused approach
  • Ability to travel across the county to support service users
