Social Worker – Prevention & Personalisation

Our client, based in Derbyshire, is seeking an experienced Adult Social Worker to join their Prevention and Personalisation Team. This role focuses on early intervention, strengths-based practice, and promoting independence, supporting adults to live well within their communities. You will deliver personalised, outcome-focused support in line with the Care Act 2014, working with a range of needs while effectively managing risk.

Key Responsibilities

Core duties include:

  • Managing a caseload of adults, focusing on prevention, early intervention, and personalised support planning.
  • Undertaking Care Act 2014 assessments, strengths-based reviews, and outcome-focused support plans.
  • Promoting independence through reablement, assistive technology, and community-based solutions.
  • Supporting individuals to access local community resources, voluntary sector services, and informal support networks.
  • Carrying out proportionate risk assessments and risk management, balancing empowerment with safeguarding.
  • Completing Mental Capacity Assessments and contributing to Best Interests decision-making where required.
  • Working within key legislative frameworks including:
    • Care Act 2014
    • Mental Capacity Act 2005 (MCA)
    • Human Rights Act 1998 (HRA)
  • Leading and contributing to Safeguarding Adults processes, including Section 42 enquiries where required.
  • Applying a strengths-based and person-centred approach to all interventions.
  • Working collaboratively across multi-disciplinary teams (MDT), including health professionals, housing, and voluntary organisations.
  • Supporting hospital discharges and prevention of admission where appropriate.
  • Maintaining accurate, timely, and high-quality case records in line with statutory requirements.
  • Demonstrating professional curiosity, sound judgement, and evidence-based decision-making.

Qualifications & Experience

Professional Qualification: MA, BA, BSc in Social Work, Diploma in Social Work, or CQSW

Registration: Must be registered with Social Work England

Experience:

  • Minimum 2 years’ post-qualified experience, including completion of ASYE
  • Strong background within Adult Social Care
  • Experience in strengths-based practice, prevention, and care management
  • Experience undertaking Care Act assessments, reviews, and support planning
  • Experience in safeguarding and multi-agency working
  • Experience supporting individuals to access community-based services and early intervention support

Legislation Knowledge:

Strong working knowledge of:

  • Care Act 2014
  • Mental Capacity Act 2005 (MCA)
  • Human Rights Act 1998 (HRA)
  • Safeguarding Adults legislation (including Section 42)

Skills:

  • Strong strengths-based assessment and person-centred planning skills
  • Ability to manage caseloads with a focus on prevention and independence outcomes
  • Excellent communication and interpersonal skills
  • Ability to work collaboratively within multi-agency environments
  • High standard of report writing and case recording
  • Good understanding of community resources and partnership working
  • Strong organisational skills and ability to prioritise workload
  • Demonstrates resilience and a proactive, solution-focused approach
  • Ability to apply professional judgement in complex situations

Other Requirements: Full UK driving licence and access to a car is desirable
