Occupational Therapist – Discharge to Assess
Liquid Personnel is seeking an experienced Occupational Therapist to support the delivery of adult social care through timely and effective initial assessments for its client based in London Borough of Tower Hamlets.
The Role:
As an Occupational Therapist, you will lead therapy assessments and rehabilitation for patients admitted to the service, primarily following discharge from hospital.
You'll undertake access visits to patients' homes, recommending minor adaptations and equipment to support safe discharge, while working closely with multidisciplinary professionals to ensure seamless transitions of care.
There may also be opportunities to support the wider Discharge to Assess pathway by managing additional referrals when required.
What will your responsibilities be?
- Lead Occupational Therapy assessments and rehabilitation programmes.
- Assess patients admitted from acute hospital settings.
- Carry out home access visits to determine equipment and minor adaptation needs.
- Manage a varied caseload and delegate work to Therapy Assistants where appropriate.
- Support the wider Discharge to Assess pathway as required.
- Work collaboratively with multidisciplinary professionals to facilitate safe, timely discharges.
- Produce accurate assessments, reports, and documentation.
- Promote independence and person-centred care throughout the discharge process.

Qualifications & Experience:
- Qualified Occupational Therapist with current HCPC registration.
- Experience within rehabilitation, discharge planning, or community Occupational Therapy.
- Strong assessment and clinical reasoning skills.
- Experience managing a caseload and supervising or delegating to Therapy Assistants.
- Excellent communication and multidisciplinary working skills.
- Full UK driving licence and access to a vehicle with appropriate business insurance is essential.
