Adults' Social Worker - Locality & Community Team

Are you an experienced Adults Social Worker looking for your next contract opportunity within a supportive and forward-thinking local authority?

Herefordshire Council is seeking dedicated and passionate Social Workers to join their Locality/Community Team. This is a fantastic opportunity to work within a strengths-based service, supporting adults to maintain their independence and achieve positive outcomes within their local communities.

The Role

You will manage a varied caseload and undertake a broad range of adult social work duties, including:

• Care Act Assessments and Reviews
• Person-centred Support Planning
• Mental Capacity Assessments
• Section 42 Safeguarding Enquiries
• Continuing Healthcare (CHC) and Section 117 work
• Partnership working with health professionals and external agencies
• Promoting independence through a strengths-based approach
• Supporting adults within the community to access the right services and support networks

You will work with a range of service users, including:

• Older Adults
• Adults with Physical Disabilities
• Adults with Learning Disabilities
• Adults with Mental Health Needs

About You

To be considered for this role, you will have:

• Social Work England registration
• A recognised Social Work qualification
• Strong knowledge of current adult social care legislation and guidance
• Experience undertaking assessments, reviews, and care management responsibilities
• Excellent communication and partnership-working skills
• A full UK driving licence
• A commitment to delivering high-quality, person-centred practice
