Social Worker - Older Adults Reviewing Team

Liquid Personnel is recruiting Social Worker for its client's Older Adults Reviewing Team at Ceredigion.
This is an excellent opportunity to join a friendly, fully staffed, and well-established team. Due to an increase in demand, the service has developed a backlog of Care and Support Plan reviews and is seeking an experienced Social Worker to provide additional capacity.

What will your responsibilities be?

• Reviewing and updating Care and Support Plans for older adults
• Undertaking assessments and reviews in line with the Social Services and Well-being (Wales) Act 2014
• Managing a caseload of older adults with varying levels of need
• Completing safeguarding enquiries where required
• Working closely with families, healthcare professionals, and partner agencies
• Promoting independence and positive outcomes for service users
• Completing all associated statutory social work duties

About the Team

The team is fully staffed and well-established, with an increased volume of work creating a backlog of reviews that require allocation. This role will focus primarily on reviewing existing Care and Support Plans and helping the service manage demand effectively.

Qualifications & Experience:

To be successful in this role you must have, 

• Qualified Social Worker
• Social Care Wales registration
• Previous experience within Adult Services
• Strong knowledge of the Social Services and Well-being (Wales) Act 2014 and safeguarding legislation
• Ability to work independently and manage workload effectively
