Social Worker - Community Health & Social Care
Liquid Personnel is recruiting Social Worker for its client's Community Health & Social Care Team at Devon.
You will manage a varied caseload of adults with complex needs, delivering high-quality, person-centred social work practice. Working closely with health colleagues, carers, families, and partner agencies, you will play a key role in safeguarding individuals and promoting independence within the community.
What will your responsibilities be?
- Undertake strengths-based assessments and develop personalised care and support plans
- Manage a varied and complex caseload
- Complete safeguarding enquiries and risk assessments
- Apply legislation including the Care Act 2014 and Mental Capacity Act 2005
- Work collaboratively with health professionals and partner agencies
- Maintain accurate and timely case recordings
- Make informed, defensible decisions that deliver positive outcomes

Qualifications & Experience:
To be successful in this role you must have,
- Social Work England registered Social Worker
- Relevant experience within Adult Social Care
- Strong assessment, safeguarding, and risk management skills
- Ability to work independently and as part of a multidisciplinary team
- Excellent communication and relationship-building skills
