Social Worker - Disabilities Team
Liquid Personnel is recruiting Social Worker for its client's Disabilities Team at Nottingham.
This is an excellent opportunity for a dedicated practitioner to support adults and young people with learning disabilities, physical disabilities, neurological conditions, and complex support needs to achieve positive outcomes, maximise independence, and maintain choice and control over their lives.
The successful candidate will manage a varied and complex caseload, undertaking strengths-based assessments, safeguarding interventions, care planning, and reviews. Working closely with individuals, carers, families, health professionals, education services, providers, and partner agencies, you will play a key role in delivering person-centred support that promotes wellbeing, inclusion, resilience, and independence.
This role requires an experienced Social Worker with strong knowledge of Adult Social Care legislation, safeguarding, transition planning, and multidisciplinary working, alongside the ability to manage complex risk and support individuals through significant life changes.
What will your responsibilities be?
- Managing a complex caseload of adults and young people with learning disabilities, physical disabilities, neurological conditions, and multiple support needs.
- Undertaking strengths-based assessments of need, risk, and eligibility in accordance with the Care Act 2014 and relevant legislation.
- Developing, implementing, monitoring, and reviewing person-centred Care and Support Plans that promote independence, wellbeing, choice, and control.
- Completing Mental Capacity Assessments and facilitating Best Interest decision-making in accordance with the Mental Capacity Act 2005.
- Undertaking Continuing Healthcare (CHC) assessments and contributing to multidisciplinary assessment and decision-making processes.
- Supporting effective transition planning for young people moving from Children's Services into Adult Social Care.
- Leading and undertaking safeguarding enquiries under Section 42 of the Care Act 2014.
- Assessing and managing complex risks, balancing duty of care with individuals' rights to make informed choices and decisions.
- Working collaboratively with individuals, carers, providers, families, and partner agencies to safeguard vulnerable adults from abuse, neglect, and exploitation.
- Preparing reports and providing evidence for safeguarding meetings, reviews, funding panels, and legal proceedings where required.
- Building positive and effective relationships with individuals, carers, families, advocates, and support networks.
- Undertaking direct work and interventions that promote empowerment, resilience, independence, and community inclusion.
- Supporting individuals experiencing significant life changes, deterioration in health, crisis situations, or changes in care arrangements.
- Advocating on behalf of individuals to ensure their wishes, views, and rights remain central to decision-making.
- Working collaboratively with health services, housing providers, education services, voluntary organisations, and community resources to improve outcomes.
- Participating in multidisciplinary meetings and contributing professional social work expertise to inform decision-making and support planning.
- Maintaining accurate, timely, and legally compliant assessments, reports, reviews, and case records.
Qualifications & Experience:
To be successful in this role you must have,
- Professional Qualification: MA, BA, BSc in Social Work, Diploma in Social Work, CQSW, CSS, or equivalent recognised Social Work qualification.
- Registration: Must be registered with Social Work England.
- Experience: Significant post-qualified experience within Adult Social Care services.
- Disabilities Experience: Experience supporting adults and young people with learning disabilities, physical disabilities, neurological conditions, and complex care and support needs.
- Legislation Knowledge: Strong understanding of the Care Act 2014, Mental Capacity Act 2005, Continuing Healthcare processes, Adult Safeguarding legislation, and associated statutory guidance.
- Assessment Skills: Proven ability to undertake strengths-based assessments, reviews, support planning, and risk management interventions.
- Safeguarding Experience: Experience undertaking safeguarding enquiries and managing complex safeguarding concerns.
- Transition Experience: Experience supporting young people through transition into Adult Social Care is highly desirable.
- Multi-Agency Working: Strong experience working collaboratively with health services, education providers, housing services, carers, providers, and partner agencies.
- Communication Skills: Excellent communication, report-writing, analytical, negotiation, and decision-making skills.
- Professional Practice: Commitment to strengths-based, person-centred practice and continuous professional development.
- Driving Licence: Full UK driving licence, access to a vehicle, and willingness to travel both within and outside the local area as required.
