Social Worker – Hospital Social Work
Liquid Personnel is recruiting an Adult’s Social Worker to join its client’s Hospital team, based in Bournemouth. This is an excellent opportunity for a dedicated practitioner to work within a busy acute hospital setting, supporting adults with complex care and support needs through safe, timely, and person-centred discharge planning.
The successful candidate will work closely with health professionals, carers, families, community services, and partner agencies to facilitate effective hospital discharge and promote positive outcomes. You will undertake strengths-based assessments, coordinate support arrangements, and help individuals maximise their independence and wellbeing following hospital admission.
This role is ideally suited to a Social Worker with recent hospital social work experience and a strong understanding of Discharge to Assess pathways, Adult Social Care legislation, and multidisciplinary working.
What will your responsibilities be?
- Managing a caseload of adults requiring social care intervention within an acute hospital setting.
- Undertaking strengths-based assessments to identify needs, risks, strengths, and desired outcomes.
- Completing Care Act 2014 assessments, reviews, and support planning to facilitate safe and effective discharge arrangements.
- Supporting complex discharge planning and ensuring appropriate services and support are in place following discharge from hospital.
- Working within Discharge to Assess pathways to promote independence and reduce reliance on long-term support where appropriate.
- Undertaking Mental Capacity Assessments and facilitating Best Interest decision-making in accordance with the Mental Capacity Act 2005.
- Leading and contributing to safeguarding enquiries in line with Adult Safeguarding legislation and local procedures.
- Assessing and managing risks effectively, balancing safety, choice, independence, and positive risk-taking.
- Working collaboratively with health professionals, therapists, discharge coordinators, community services, carers, families, and partner agencies.
- Arranging and coordinating care packages, reablement services, community support, and placements where required.
- Participating in multidisciplinary meetings, discharge planning meetings, safeguarding discussions, and case reviews.
- Preparing high-quality assessments, reports, support plans, and professional documentation.
- Maintaining accurate, timely, and legally compliant case records in accordance with statutory and organisational requirements.
Qualifications and Experience:
To be successful in this role you must have;
- Professional Qualification: MA, BA, BSc in Social Work, Diploma in Social Work, CQSW, CSS, or equivalent recognised Social Work qualification.
- Registration: Must be registered with Social Work England.
- Experience: Significant post-qualified experience within Adult Social Care services.
- Hospital Experience: Recent experience working within a Hospital Social Work Team, Hospital Discharge Team, or Discharge to Assess service is highly desirable.
- Legislation Knowledge: Strong understanding of the Care Act 2014, Mental Capacity Act 2005, Adult Safeguarding legislation, and associated statutory guidance.
- Discharge Planning Experience: Experience supporting complex discharge planning and hospital discharge pathways.
- Assessment Skills: Proven ability to undertake complex assessments, support planning, risk management, and safeguarding interventions.
- Safeguarding Experience: Experience undertaking safeguarding enquiries and managing safeguarding concerns involving vulnerable adults.
- Multi-Agency Working: Strong experience working collaboratively with health professionals, community teams, care providers, carers, families, and partner agencies.
- Communication Skills: Excellent communication, report-writing, analytical, negotiation, and decision-making skills.
- Professional Practice: Commitment to strengths-based, person-centred practice and continuous professional development.
