Social Worker – Community Social Work

Swindon

Our client based in Swindon is seeking an experienced Social Worker to join their Community Social Work Team. This is an excellent opportunity for a dedicated practitioner to support adults with a range of care and support needs through strengths-based, person-centred social work practice.

The successful candidate will manage a varied and complex caseload, undertaking statutory assessments, safeguarding interventions, support planning, and reviews. Working closely with individuals, carers, families, health professionals, providers, and partner agencies, you will promote independence, wellbeing, and positive outcomes whilst ensuring people remain at the centre of all decision-making.

This role requires a confident Social Worker who can work autonomously, manage risk effectively, and contribute to high-quality Adult Social Care practice within a supportive multidisciplinary environment.

What will your responsibilities be?

  • Managing a complex caseload of adults with diverse care and support needs across the community.

  • Developing, implementing, and reviewing person-centred Care and Support Plans that promote independence, wellbeing, and choice.
  • Undertaking Mental Capacity Assessments and facilitating Best Interest decision-making in accordance with the Mental Capacity Act 2005.
  • Leading and contributing to safeguarding enquiries, including Section 42 enquiries, and supporting adults at risk of abuse, neglect, or exploitation.
  • Assessing and managing complex risks whilst balancing individual rights, choice, control, and safety.
  • Working collaboratively with health services, housing providers, care providers, families, carers, and partner agencies to coordinate effective support.
  • Undertaking Court of Protection work, preparing reports, and contributing to legal processes where required.
  • Supporting carers through assessment, advice, guidance, and access to appropriate services and support.
  • Acting as an advocate for individuals and ensuring their voice, wishes, and outcomes remain central to all interventions.
  • Maintaining accurate, timely, and legally compliant records, assessments, reports, and case documentation.
  • Providing professional advice and guidance to colleagues and contributing to service development and continuous improvement initiatives.

Qualifications & Experience:

To be successful in this role you must have:

  • Professional Qualification: MA, BA, BSc in Social Work, Diploma in Social Work, CQSW, CSS, or equivalent recognised Social Work qualification.
  • Registration: Must be registered with Social Work England.
  • Experience: Significant post-qualified experience within Adult Social Care services.
  • Legislation Knowledge: Strong understanding of the Care Act 2014, Mental Capacity Act 2005, Adult Safeguarding legislation, Court of Protection processes, and associated statutory guidance.
  • Assessment Skills: Proven ability to undertake complex assessments, reviews, support planning, and risk management interventions.
