  • Supporting individuals to access community resources, direct payments, supported living services, and other appropriate support arrangements.
  • Working collaboratively with health professionals, housing providers, care providers, carers, advocacy services, and partner agencies to achieve positive outcomes.
  • Coordinating complex care and support arrangements and monitoring the effectiveness of services provided.
  • Supporting individuals through periods of crisis, transition, deterioration in health, or changes to support arrangements.
  • Participating in multidisciplinary meetings, safeguarding discussions, reviews, and case conferences.
  • Maintaining accurate, timely, and legally compliant records, assessments, reports, and support plans.
  • Promoting strengths-based practice and empowering individuals to exercise choice, control, and independence in their daily lives.
