Social Worker - Community Prevention

Nottingham

Liquid Personnel is recruiting Social Worker for its client's Community Prevention Team at Nottingham.

Our client based in Nottingham is seeking an experienced Social Worker to join their Community Prevention Team. This is an excellent opportunity for a skilled practitioner to support adults with complex care and support needs through preventative, strengths-based interventions that promote independence, wellbeing, and positive outcomes.

The successful candidate will manage a varied and complex caseload, undertaking Care Act assessments, reviews, support planning, safeguarding enquiries, and Mental Capacity Act work. Working closely with citizens, carers, health professionals, and partner agencies, you will play a key role in preventing escalation of needs, reducing reliance on long-term services, and supporting individuals to remain independent within their communities.

This role requires a confident Social Worker with strong assessment and analytical skills, experience of Continuing Healthcare processes, and the ability to work effectively within a multidisciplinary environment. The role offers a hybrid working arrangement, combining community visits, home working, and office-based practice.

What will your responsibilities be?

  • Managing a complex caseload of adults with a range of care and support needs within the Community Prevention Team.
  • Undertaking Care Act 2014 assessments, reviews, reassessments, and support planning to identify eligible needs and promote positive outcomes.
  • Developing, implementing, and reviewing person-centred Care and Support Plans that maximise independence, wellbeing, and resilience.
  • Undertaking safeguarding enquiries and responding appropriately to safeguarding concerns in accordance with Adult Safeguarding legislation and local procedures.
  • Completing Mental Capacity Assessments and facilitating Best Interest decision-making in accordance with the Mental Capacity Act 2005.
  • Supporting Continuing Healthcare (CHC) processes, including completion of CHC assessments and participation in Decision Support Tool (DST) meetings.
  • Assessing and managing complex risks whilst promoting positive risk-taking and strengths-based practice.
  • Working directly with citizens and carers to identify strengths, community resources, and sustainable support networks.
  • Collaborating with health professionals, community services, care providers, housing services, and partner agencies to coordinate effective support arrangements.
  • Promoting preventative interventions that reduce the need for long-term statutory services where appropriate.
  • Participating in multidisciplinary meetings, safeguarding discussions, CHC reviews, and case conferences.
  • Preparing high-quality assessments, reports, professional recommendations, and support plans.
  • Maintaining accurate, timely, and legally compliant case records and documentation.
