Social Worker - Transitions

Liquid Personnel is recruiting an Adult’s Social Worker to join its client’s Transitions team, based in North Lincolnshire. 

What will your responsibilities be? 

• Undertake assessments of young people’s care and support needs as they transition into adulthood.
• Develop and review person-centred transition plans in partnership with young people, families and carers.
• Promote independence, choice, control and preparation for adult life.
• Work collaboratively with children’s services, adult social care, education, health and other partner agencies.
• Ensure timely and effective transitions from children’s services to adult services.
• Assess and manage risks, safeguarding concerns and vulnerabilities.
• Undertake Care Act assessments and mental capacity assessments where appropriate.
• Support access to suitable accommodation, education, employment, community services and independent living opportunities.
• Provide advice, guidance and support to young people and their families throughout the transition process.
• Ensure compliance with relevant legislation, including the Care Act 2014, Children and Families Act 2014 and Mental Capacity Act 2005.
• Maintain accurate case records, assessments, reports and transition plans.
• Participate in multi-agency meetings, supervision and service reviews to ensure effective outcomes for young people.

Qualifications and Experience: 

To be successful in this role you must have;  

• Social Work England registration 
• Eligible to work in the UK 
• Hold a full UK driving licence 
• Post-qualified experience 
• Degree level or equivalent in Social Work
