Schedule:
Varied per‑diem hours, Monday–Friday between 8:30 AM–5:00 PM EST
Occasional evening hours may be required

Location & Travel:
Aligned to our Elmsford, NY location
Travel within a two (2)‑hour radius of the greater metropolitan area may be required

Our nurses play a critical role in helping patients transition confidently from hospital to home while managing complex therapies with skill and compassion.

We’re looking for a Home Infusion Nurse (Per Diem) who wants their work to mean something — a Registered Nurse who brings clinical expertise, independence, and heart to home‑based infusion and nutrition therapy.

Home infusion and nutritional support require a high level of clinical judgment, trust, and autonomy. As a Home Infusion Nurse, you are often the primary clinical connection for patients navigating therapy outside the hospital setting.

In this role, you’ll contribute to work that:

  • Helps patients safely manage IV and nutritional therapies at home
  • Supports smooth transitions from acute care to home‑based treatment
  • Builds confidence through education, assessment, and ongoing monitoring
  • Strengthens a patient‑first, clinician‑led approach to infusion care

Your presence helps ensure patients feel supported, informed, and capable throughout their care journey.


What You’ll Do

In this role, you will:

  • Deliver high‑quality home‑based nursing care to patients receiving IV therapy and nutritional support
  • Perform comprehensive patient evaluations, including physical and nutritional assessments
  • Administer and monitor intravenous solutions and IV/IM/SubQ medications per physician orders
  • Initiate and manage peripheral IVs and central venous access devices; obtain labs via venipuncture or central access
  • Monitor patients for mechanical, infectious, metabolic, and pharmacological complications
  • Provide hands‑on patient and caregiver education for infusion self‑management and therapy adherence
  • Support hospital‑to‑home transitions by collaborating with physicians and healthcare teams
  • Maintain thorough, accurate documentation of visits, procedures, and patient communications
  • Coordinate equipment, supplies, and additional in‑home services as needed
  • Participate in patient care conferences, quality initiatives, infection control, and emergency preparedness activities
  • Serve as case manager for assigned nursing and therapy patients
  • Support onboarding and training of new Registered Nurses when requested

Required Qualifications

  • Graduate of an NLN‑accredited school of nursing
  • Current, unrestricted Registered Nurse (RN) license in New York State
  • Minimum of one (1) year of IV therapy experience with strong background in nutritional support and/or immunoglobulin therapy
    or
    Two (2) years of recent acute care experience (or completion of a supervised apprenticeship for home visits)
  • CPR certification (or ability to obtain within 30 days of hire)
  • Strong technical knowledge of clinical nutrition and IV therapy
  • Valid NYS driver’s license and ability to travel to patient homes
  • Basic computer proficiency and electronic documentation skills
  • Dependable attendance and professional reliability

Preferred Qualifications

  • Prior home infusion nursing experience
  • Participation or membership in professional nursing organizations

BioMatrix Pay Range/Rate

$45 - $55 USD
