Engagement Manager (Sonora)

Position Summary:

Manages community engagement in Life Center, as part of a team of wellness and engagement professionals who facilitate a variety of programs, events, partnerships, and more. Key areas of managerial responsibility include implementing a robust calendar of group and individual programs that support high levels of resident engagement in Assisted Living, Memory Support, and Skilled Nursing. Implements Culture of Creativity and ContinuWell within Life Center, ensuring that “residents are creators, not just consumers of culture- participants in the human experience, not just recipients of care”. Oversees Life Center Calendar development and communications, Life Center volunteer management, Life Center engagement budget, independent contractors/partnerships, engagement regulatory compliance, and outcomes measurement.

ESSENTIAL FUNCTIONS:

    • Delivers programs for Life Center residents following Culture of Creativity model: including best practices for group and 1:1 engagement with music, movement, storytelling/legacy, nature immersion, culinary arts, sensory experience, inquiry-based learning, and meaningful celebrations of holidays & rituals within daily routine. Implements best practices for engaging residents with diverse physical, cognitive and emotional needs.
    • Manages calendar development for Life Center programs meeting Mather standards for engagement and wellness: and ensures that programs occur as scheduled via personally facilitating programs & supporting programs led by colleagues, independent contractors and volunteers. Contributes ideas to support program development across the continuum. Facilitates outings/events and participation of Life Center residents in broader community events, and amenities.
    • Ensure completion of required assessment processes for regulatory compliance including care plan notes and documentation within the EMR Supports MDS process as required. Translates residents’ personal interests and needs developed within assessment and care plan procedures into unique individual and group engagement opportunities and programs within the Life Center calendar. ‘Attends care plan meetings as directed.
    • Trains, creates schedules, and performance manages engagement specialists. Provides daily supervision of engagement specialists including day-to-day coaching in program delivery, resident engagement, and assessment/documentation completion.
    • Contributes to inviting, inclusive, and uplifting culture and therapeutic environment in Life Center
    • Identifies ways to engage with residents regularly to seek input and feedforward: Maintains regular communications with residents, family members and staff.

    • Ensures adherence to state and federal regulations for Life Center programs & Mather Culture of Creativity & Creating My Possibilities (CMP) Maintains frequent communication with Life Center Administrator. Manages completion of transition recreational/leisure assessments via personal completion of assessments with residents or assignment of team members. Ensures that engagement team has training/best practices/skills for therapeutic programs, resident engagement, designing programs, individual and group experiences, signature programs, etc.
    • Manages Life Center engagement budget & independent contractor contracts/billing. 
    • Maintains strong relationships with residents and family members including coordination of/representation in resident committees and resident councils across the Identifies ways to engage with residents and families regularly to seek input and feedforward. Supports Director of Resident Engagement in gathering resident satisfaction and engagement data.
    • Manages community volunteer program: Provides volunteer orientation, scheduling, and ongoing guidance for Mather resident volunteers across the continuum. Responsible for compliance with all Federal, State and Mather volunteer policies and procedures.
    • Participates in strategic planning with Director of Resident Engagement and SL leadership to develop programs driven by Mather brand standards for resident engagement and wellness. Ensures successful delivery of Signature Programs within community.
    • Establishes, operationalizes, engages team members in, and sustains a Quality Assurance Process
    • Improvement process.
    • Participates in community Creating My Possibilities Champions team, ensuring implementation of strengths-based experiences for residents living with dementia in line with CMP model, serving as a co-trainer facilitating workshops for employees, IL residents, and families.

     

    Secondary Functions:

    • Creates communication materials for upcoming programs and tracks participation and feedback
    • Reports incidents, ensures compliance with safety and risk management for program staff
    • Facilitates family engagement opportunities to foster relationships and support resident family Maintains respectful and welcoming communication with family members through verbal and written communications and connects families with resources within broader interdisciplinary team to support meaningful engagement.
    • Creates relationships based on respect, using a customer-focused approach to problem solving, maintaining a courteous and professional manner through interactions with others. Communicates with key staff, residents and families in a timely manner. Maintains respectful and communicative relationships with interdisciplinary team, ensuring collaboration to support resident engagement.

     

    • Attends in-services, care plan meetings, committees/resident council, and participates in QAPI processes regarding resident engagement.
    • Complies with Mather Code of Conduct, Policies, Compliance Program, and all applicable laws and regulations in performance of one’s job duties, including meeting state and federal regulations regarding resident engagement in SN and AL settings; reports all known and suspected violations to his/her supervisor or manager, Mather Compliance officer, a Human Resources representative, or the confidential employee
    • Is part of an interdisciplinary team that supports the resident journey from the pre-entry screening to, if necessary, transitions into the Life Center.
    • Demonstrates understanding of and commitment to the company Mission, Core Values, and EMI
    • Uses discretion in handling confidential information, incorporating all Federal, State and local privacy and confidentiality Ensures Mather consent protocols are followed across continuum for resident content.

    Qualifications and Skills:

    Required:

    • College Degree in Recreation Therapy, Creative Arts Therapies, Humanities, Geriatrics or related field, or 3+ years related experience.
    • Excellent customer service and written/verbal communication
    • Positive, engaging, and collaborative attitude, strong 1:1 and group facilitation
    • Ability to lead by example; self-initiator requiring minimum
    • Experience developing engaging programs around Health and Wellbeing, Lifelong Learning, and Arts and Culture for adults with various physical and cognitive impairments.
    • Knowledge of person-centered
    • Proficiency in MS Publisher, MS Word, MS Outlook, MS Excel and PCC or other related EMR and demonstrated proficiency at documenting to meet regulatory compliance standards.
    • Ability to use or learn a variety of tech-based services and platforms; e. Microsoft Outlook, Zoom, YouTube, resident facing portals, and A/V set-ups.

    Preferred:

    • Strong creative abilities related to diverse forms of expression (music, movement, improv, art, )
    • Knowledge of state and federal regulations pertaining to Medicare/LTC. Experience with care plan and MDS documentation relating to resident activity program requirements.
    • 36 / 42 Hour Activity Director Course completion or related as required by federal/state Medicare

This position involved primarily light work, frequently lifting up to 10 pounds; frequent standing/walking
