Position Details:
The Clinical Director of Behavioral Health Integration (BHI) and Collaborative Care Management (CoCM) will be based in Hollywood, FL. This senior role is responsible for leading and advancing the Chronic Care and Population Health Medicine division, which encompasses CCM, RPM, PCM, Genomics, and other relevant programs including BHI, CoCM, and Neuro-Cognitive care. The Clinical Director will oversee the clinical and operational aspects of the division, ensuring high-quality care and effective team management.
Responsibilities:
Leadership and Advancement:
- Lead and advance the Chronic Care and Population Health Medicine division, including CCM, BHI, CoCM, and Neuro-Cognitive programs.
- Provide clinical leadership and structure to the current BHI/CoCM team.
- Contribute to policy and procedure changes, ensuring alignment with best practices and organizational goals.
Team Building and Oversight:
- Build and maintain high-performing teams through effective recruitment, training, and professional development.
- Oversee PODS assigned to ensure they meet performance standards and deliver quality care.
- Conduct weekly meetings with PODS for education and team alignment.
- Lead daily huddles with staff to ensure effective communication and coordination.
Quality Assurance:
- Conduct quality assurance activities, including chart audits and ongoing evaluation of clinical practices.
- Implement and monitor quality improvement initiatives to enhance care delivery.
Coordination and Collaboration:
- Attend Population Health Management (PHM) meetings as requested by facilities overseeing PODS.
- Work closely with Operations for scheduling and coordination with BHCM and Psych team members.
- Conduct meetings with facilities and corporate teams to discuss program performance and improvements.
- Hold weekly meetings with the Chief Medical Officer (CMO) to review progress and strategic initiatives.
Requirements:
- Advanced clinical degree (e.g., Doctor of Medicine, Doctor of Osteopathy, or Master’s in Nursing with appropriate certification).
- Board certification in a relevant specialty (e.g., Internal Medicine, Psychiatry, or Family Medicine).
- Proven experience in chronic care management, behavioral health integration, and collaborative care models.
- Strong leadership skills with experience in team management, quality assurance, and operational oversight.
- Excellent communication and interpersonal skills, with the ability to collaborate effectively with diverse teams and stakeholders.
- Experience with Population Health Management (PHM) and familiarity with CCM, RPM, PCM, and Genomics programs.
- Ability to adapt to changing environments and lead continuous improvement initiatives.

Salary Range: $110,000.00 -140,000.00 per year
