Job Summary
The Manager of Behavioral Health Integration (BHI) and Collaborative Care Management (CoCM) plays a key supervisory role within the Chronic Care and Population Health Medicine division. This position is responsible for supervising the day-to-day functions of the BHI/CoCM team, ensuring clinical workflows are followed, and quality standards are maintained. The Manager acts as a conduit between frontline team members and leadership, facilitating operational alignment and supporting professional development across the team..
Responsibilities:
Team Supervision and Support:
- Supervise BHI/CoCM team members to ensure adherence to program protocols and clinical workflows
- Conduct daily check-ins and team huddles to review caseloads, address challenges, and reinforce best practices
- Support onboarding and ongoing training efforts for BHI/CoCM staff in alignment with organizational procedures
- Serve as the first point of contact for staff questions, scheduling needs, or workflow concerns
Performance Oversight:
- Monitor individual and team performance metrics to ensure compliance with quality and productivity standards
- Conduct chart reviews and assist in implementing corrective action plans when needed
- Coordinate with the Clinical Director to address performance trends and improvement areas
Operational Coordination:
- Support communication across multidisciplinary teams, including BHCMs and psychiatric consultants
- Attend Population Health Management (PHM) meetings as assigned, representing the operational needs and updates of the BHI/CoCM team
- Collaborate with the Operations team to ensure proper allocation of resources and continuity of care
Quality Improvement and Compliance:
- Assist in executing quality assurance initiatives under the guidance of the Clinical Director
- Ensure that clinical documentation meets compliance standards and supports billing practices
- Identify workflow gaps or inefficiencies and escalate appropriately for resolution
Requirements
- Bachelor’s or Master’s degree in Nursing
- Minimum of 2–3 years of experience in behavioral health, population health, or chronic care management settings
- Strong leadership skills with experience in team management, quality assurance, and operational oversight
- Familiarity with Collaborative Care Model (CoCM), BHI practices, and population health workflows preferred
- Excellent organizational, interpersonal, and communication skills
- Proficiency with electronic health records (EHR) and data tracking systems
- Ability to adapt to changing environments and lead continuous improvement initiatives.
