Job Summary
The Manager of CoCM Clinical Coordination plays a key leadership role in ensuring the accuracy, compliance, and overall performance of clinical documentation and care workflows across our Collaborative Care Model (CoCM) programs. This role is responsible for overseeing a team of Clinical Coordinators, conducting regular audits, identifying training needs, and driving quality assurance processes in alignment with regulatory and organizational standards. The ideal candidate brings strong clinical judgment, proven leadership skills, and a deep understanding of documentation standards within post-acute, long-term care, and behavioral health settings.
Key Responsibilities:
- Supervise and support a team of Clinical Coordinators, serving as the primary point of contact for performance oversight, scheduling, training, and case escalations
- Lead ongoing audits of documentation across CoCM and other behavioral health programs to ensure regulatory compliance, clinical quality, and billing accuracy
- Identify trends, documentation errors, and workflow gaps, and implement corrective action plans or additional staff training as needed
- Maintain up-to-date knowledge of CMS, and all other regulatory standards related to long-term care and behavioral health integration
- Monitor KPIs and team productivity using dashboards, audit tools, and quality metrics; provide regular reports to senior leadership
- Collaborate with behavioral health consultants, psychiatrists, and interdisciplinary teams to ensure clinical integrity and adherence to collaborative care protocols
- Serve as an escalation resource for complex cases, staff questions, or EMR-related issues
- Participate in hiring, onboarding, and professional development of Clinical Coordinators
- Champion a culture of compliance, clinical excellence, and accountability
Required Skills & Experience:
- Registered Nurse (RN) with an active license in good standing
- Proven leadership or supervisory experience in clinical or QA roles
- Experience in Behavioral Health, Psychology and Psych Medications knowledge/experience is strongly preferred
- Long-Term Care or Post-Acute Experience (SNFs, ALFs, or similar settings)
- Strong background in chart audits, quality assurance, and compliance reviews
- Working knowledge of ICD-10, CPT codes, and medical billing practices
- EMR proficiency (PointClickCare, MatrixCare, or similar platforms)
- Familiarity with CMS CoCM documentation standards, is a plus, including validated tools (e.g., PHQ-9, GAD-7), care plans, and progress notes
- Excellent written communication, organization, and clinical documentation review skills
- Strong process orientation and ability to train others on workflow consistency and documentation accuracy
Preferred Backgrounds:
- QA Managers or Lead Auditors in Long-Term Care or Behavioral Health Settings
- Former Home Health, Hospice, or ALF QA Staff with team oversight responsibilities
- Utilization Review or Discharge Planning RNs from Hospital or Insurance backgrounds
- Behavioral Health or Integrated Care RNs with experience managing collaborative care teams
- Nurses with leadership experience across multiple facilities or regions (e.g., travel RNs in supervisory roles)
