JOB SUMMARY
The Senior Lead Patient Navigator provides case management and care coordination as part of a multidisciplinary clinical team, which includes: Navigator, Nurse, Behavioral Health Medical Professional (BHMP), Patient Advocate and Patient Care Coordinators under the direction of Assistant General Manager. The Senior Lead Patient Navigator will be responsible for coordination of care activities, follow up activities, resource development, assessment, and treatment planning, staffing cases with clinical team members and discharge planning. Additionally, the Senior Lead Patient Navigator will train team members on coordination of care activities, follow up activities, resource development, assessment, and treatment planning, staffing cases with clinical team members and discharge planning. The Senior Lead Patient Navigator will oversee Patient Navigator case management productivity. The Senior Lead Patient Navigator is a member of the shift leadership team that manages the units and patients.
ESSENTIAL JOB FUNCTIONS
- Facilitation and monitoring of daily activities and coordination of care with Crisis Observation Unit, Express and Progressions
- Communication with the clinical team to ensure client’s goals and objectives are clear
- Monitoring and coordination efforts with family members and team on service delivery methods and options
- Staffing patients with other team members
- Coordination client appointments with clinical team members
- Makes recommendations to identify how to assist patient with any social determinant needs identified
- Training new team members on the daily duties of a Patient Navigator job
- Conduct ongoing training of Patient Navigators
- Responsible for conducting chart audits of Patient Navigators, ensuring accuracy, completion and compliance
- Oversee Patient Navigator case management productivity
- Educate team members working with patient and family to educate on needed services as well as advocate for additional services identified
- Performance of case management duties as defined in each member’s individualized treatment plan
- Monitoring completion of all needed documentation for behavioral services provided
- Manage a patient caseload
- Participate in ongoing education, including in-services, training, and other activities to maintain and improve
- Demonstrate competency through post-testing, skill observation, and performance as assessed by direct supervisor.
- Will attend supervision sessions as required.
- Will complete all other required trainings as assigned.

POSITION QUALIFICATIONS
Minimum Qualifications:
- Bachelor’s degree in behavioral health or related healthcare field or High School Diploma or equivalent and 4 years of behavioral health experience
- Management or supervisory experience
- Level 1 Fingerprint Clearance Card, or ability to obtain upon hire
Preferred Qualifications:
- Master’s degree in behavioral health or related healthcare field
- Associate license of LAC, LMSW, or equivalent within the state of practice
- Understanding of Medicaid and commercial insurances in the region, and Medicare system
- Knowledge of resources in the region
- Knowledge of the Child Welfare System, Mental Health Legal System, Elder Abuse Reporting System,
- Victim Notification processes, safety planning, and ASAM/Calocus/Locus
- Prior experience providing crisis services and/or working in a psychiatric facility
- Current licensure in state which services are being rendered (LCSW, LMSW, LAC, LPC, or LMFT) and/or identified as a Clinical Supervisor
