Hospice Registered Nurse / RN On-Call (7 On/7 Off, 5PM-8AM)

- Respond promptly to after-hours calls from patients, families, caregivers, and staff.
- Provide telephone triage, symptom management, and clinical guidance.
- Determine the need for in-person visits and conduct home visits when appropriate.
- Assess patient condition and provide skilled nursing care following care plans and physician orders.
- Collaborate with on-call physicians, the interdisciplinary team, and other healthcare providers.
- Document all interactions, assessments, interventions, and follow-ups accurately and promptly.
- Communicate significant changes in patient condition to the appropriate staff or supervisor.
- Participate in handoff communication at the beginning and end of the on-call shift.
- Maintain compliance with regulatory standards, company policies, and clinical protocols. Communicates with the physician regarding the patient’s needs and reports and changes in the patient’s condition; obtains/receives physicians orders as required.
- Communicates with community health related persons to coordinate the care plan.
- Assesses patients' dietary and nutritional needs based on medical history, current health status, and treatment plans.
- Provides evidence-based dietary counseling and education tailored to individual patient needs, preferences, and cultural considerations.
- Possesses a degree from a nursing program approved by an accredited organization.
- Licensed to practice as RN within the state of practice.
- Two years of appropriate experience as a nursing Community/home health/hospice experience is preferred.
- Demonstrates good verbal and written communication and organization skills.
- Possess and maintains current BLS Hands On CPR Certification.
