Location: San Diego, CA
- Utilize available resources to identify patients at high risk for rehospitalization and mortality and provide an extra layer of support.
- Provide education and resources to patients, clinicians and community partners on available care options.
- Support patients as the navigate the healthcare continuum.
- Provide personalized advice based on the patient’s unique goals and care needs.
- Provide guidance to patients during challenging situations and decision-making processes.
- Assist with advance care planning tailored to a patient's wishes and values.
- Educate and guide patients regarding healthcare systems, including home health, hospice, palliative care, hospitalizations, and skilled nursing or rehabilitation placement.
- Maintain a strong sense of responsibility in responding to patients and clinicians, ensuring timely and effective support is provided as needed.
- Ability to maintain careful records and document all patient/family interactions.
Qualifications:
- High school graduate or equivalent required. College degree preferred.
- 3 years’ experience in health care office, preferably in hospice and/or home health care.
- Proficient computer skills required
- Demonstrates excellence in customer service and time management
- Knowledge of corporate business management, governmental regulations, CHAP standards, and private payer practices.
- Demonstrates good communications, negotiation, and public relations skills.
- Demonstrates autonomy, organization, assertiveness, flexibility, and cooperation in performing job responsibilities.
- Flexible schedule availability (based on business need)
